On April 30, 1982, members of the NRC staff met with representatives of Rochester Gas and H]ectric Corporation (RGSE) to review the steam generator evaluation r eport dated April 26, 1982.
The puvpose of the meeting was for the licensee and his consultants to summarize the results of RG5E's tube failure analysis program, present conclusions as. to the reasons for the January 25, 1982 tube rupture and to have the NRC staff identify any additional information required fov the timely completion of the Safety Evatugtion Report (SER).
Enclosure 1 is a list of attendees.
Enclosure 2 is a copy of the viewgraphs used during the presentation.
The meeting started with a history of tube plugging in the "8" steam generator hot leg.-
The progression of the degradation in wedge area 4, where the.tube rupture occurred, was highlighted.
Later in the meeting RG&E stated that they now feel that the degradation in this wedge area Iras been due to the pr~errce:- of a large foreigh object tl)at damaged the perepheral.
t'bbes.;d As the peripheral tubes were continually ijpacted by this foreign object they eventually severed at the tube sheet.
The severed tubes were then free to impact and were against the next row of tubes until sufficient damage caused these tubes to sever.
This progression can be seen in the way that tubes weve plugged through the years as ECT indications were received during the regular insservice, inspection program that RG5E performs.
During the years that the dt.'gt'adation was progressing, RGEE was investigating other modes of degradation suc) as axial loading of the-tubes in the wedge area.
Post event examiqagfon and analysis now point'airly conclusively to the presence of foreign objects.
No evidence of corrosion in any of the tube samples was found.

Also, a description of'he repair program and its basis were also presented.
The NRC staff.pierewt generally agreed with RGSE's overall analyses.
The sta,ff will continue its review of the licensee's submittal and any qgh'stions raised during than review will be resolved via con-ference calls or other meetings as necessary.
RGSE is presently proposing a start up date of Hay 19, 1982.
