Dear Dr. Murley:

,In accordance with the above subject which stated,

Appendix 8, Criterion V, "Instructions, Procedures, and Drawings" requires that activities affecting quality be prescribed by documented procedures and accomplished in accordance with these procedures.

Ginna Station Quality Assurance

Manual, Section No. 5, Paragraph 5.1" Instructions, Procedures, and Drawings"
states, in part, that "activities affecting quality will be conducted in accordance with documented instructions and procedures.

Contrary to the

above, as of February 15, 1985 it was determined that the valve preventive maintenance program reviews and valve inspections were not being accomplished as prescribed in Administrative Procedures A-1020, "Valve Preventive Maintenance Program,"

INTERIM ACTION Immediately following this inspection, Maintenance Supervision began a thorough review of Administrative Procedure A-1020. It was concluded that the procedure needed a complete revamping due to its unworkable nature.

While this was on-going, a valve work list for 1985 was developed.

This valve work list contained valves that required PM during the calendar year 1985 and is being used as a control list which contains valves scheduled to be worked on and those valves that were actually completed.

At years end, this list will be reviewed by Maintenance Supervision and submitted to Central Records as a record of valve PM accomplished.

Administrative Procedure A-1020 will be revised by December 13, 1985, to provide a more effective valve PM program.

LONG TERM ACTION The following long term action will be taken to provide for corrective action to preclude reoccurence:

full time PM Coordinator/Planner and assistant as necessary will be assigned to the Maintenance Section effective January 6,

A task force has been established at the-corporate level with Ginna Station Maintenance Supervision representation.

The task force will evaluate, formalize, and recommend a

computerized power plant maintenance information system for implementation at Ginna Station.

The results of this task force study will be completed by December 13, 1985.

Based on the final task force study and management approval, a computerized information system will be selected and implemented at Ginna Station.

This system will be used to implement the valve PM program.

It will provide for

planning, scheduling, work control, documentation history of valves in the PM program.

It is anticipated that full implementation can be achieved by December 30, 1987.
