Dear Mr. O'Leary:

In response to an off site review of Deviation Report No.D-12-3-74-10 (reference 1) the following supplemental report is issued. In the opinion of the off site review, the Evaluations section did no.t completely address adequate corrective action to prevent repetition or evaluation of safety implications in light of cumulative experience.

The revised Evaluations section follows in its entirety for pu11poses of clarity.

EVALUATIONS (Supplement to original letter)

This occurrence did not jeopardize the safety of the plant or the public and resulted in no significant contamination problems.

Precautions normally in effect for the area were sufficient.

The imnediate action instituted was satisfactory and effective.

The* leak was minor, to the extent that it was leaking leas than a bad valve packing.

The fact that it was due to a cracked weld rather than a packing resulted in its classification as an "abnormal degradation."

However, the safety implication was identical to a "normal" degradation which is not considered to have serious safety implications.

of this type on similar equipment.

This occurrence poses no reasons for limiting future operation of Dresden Unit 3.

  • Because of the nature of the (ailure, no corrective action can be.efficted to prevent repetition of the occurrence on this and *similar eguipment, Cumulative experience regarding miscellaneous contaminated water line leaks of this nature is minimal.

Because of *the.insignificance of this type of failure they were not reportable until recently,,, Thus; there is no conclusive recorded history available* at this time.
