Emergency Diesel Generators Dresden.Station

Dear Mr. Knop:

At approximately** 0845 hours0.00978 days <br />0.235 hours <br />0.0014 weeks <br />3.215225e-4 months <br /> on December 28, 1977, it was discovered that both the. Dresden. 3 and 2/3 emergency diesel generators were electrical+y disabled. This condition took away all emergency diesel generator capabilities to Unit 3. An immediate investigation *indicated that an operating error had been made at 0545 hours0.00631 days <br />0.151 hours <br />9.011243e-4 weeks <br />2.073725e-4 months <br /> of the same day when the equipment outage was hung in anticipation of taking the 2/3 diesel generator out of service for routine monthly inspection. The error was corrected at 0905 hours0.0105 days <br />0.251 hours <br />0.0015 weeks <br />3.443525e-4 months <br />.

As part of the investigation, an immediate corrective action was proposed to preC:lude similar problems. This corrective action consists of two items:

1. An operating order will be issued today (12/28/77) requiring a second individual to independently verify that equipment is taken in and out of .service correctly. This order will only apply to safety-related equipment and will include valve and electrical operations.
  • This operating order will serve as a temporary measure until item 2 has been concluded and the results of that Task Force implemented.
2. A blue-ribbon ~ask Force which has already been formed will review the incident and make recommendations to Company management to preclude.similar problems. This Task Force intends to have its first meeting at Dresden Station at 1330 hours0.0154 days <br />0.369 hours <br />0.0022 weeks <br />5.06065e-4 months <br /> on December 29, 1977.

Manager Doug Scott (Dresden SRO), Assistant Supt. of Braidwood Station D~esden Station Union Representative In addition, a meeting will be held by Station management at 1600 hours0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br /> on December 28, 1977, with the Day Shift senior reactor operators, center desk nuclear stationopei:ator, and union. steward, to discuss the seriousness of the diesel generator event and to inform the personnel of the new operating order. A similar meeting will be held at 1700 hours0.0197 days <br />0.472 hours <br />0.00281 weeks <br />6.4685e-4 months <br /> with the night shift personnel. This meeting will include personnel involved in the event and will determine the specifics surrounding it in addition to the items discussed in the previous meetingo This immediate and thorough investigation and corrective action plan should indicate our serious concerns surrounding this and similar events. Our intention is to generate a comprehensive plan which will preclude this type of occurrence which has occurred recently at the station.
