REPORT OF FOREIGN OBJECTS FOUNQ INSIQE PIPING AQJACENT TO AN OPEN VALVE About 9:00 p.m.
on March 30, the licensee informed the Senior Resident Inspector of the following:
at 5:00 p.m., with the plant shut down and the reactor defueled, two
- wrenches, a rag and an aerosol can were found in the Residual Heat Removal System suction pipe from the Refueling Mater Storage Tank (RMST).'
motor operated valve in the pipe had been opened and the internals were removed for maintenance.
The valve was reported to have satisfactorily passed a cleanliness inspection, in preparation for assembly, at 2:00 a.m. that day.
The foreign objects were discovered about four inches into the pipe toward the RMST when contractor personnel began to reassemble the valve.
Based upon the one satisfactory cleanliness inspection, the licensee believed that the.
foreign objects may have been intentionally placed into the pipe.
Increased surveillance by auxiliary operators and security personnel was initiated in areas containing critical systems and equipment, and a manual system of personnel accountability was implemented in specific work areas.
Increased monitoring of other open piping was performed.
An airborne radiological contamination control enclosure had been erected at the work area and respiratory protection was required for access.
According to radiation work permit records, only six individuals had access to the work area.
Licensee and local law enforcement (LLEA) investigators interviewed the six individuals during the morning of March 31 and concluded that the individual who reported finding the foreign material had fabricated the story.
The contract employee is expected to be discharged.
This PN is being issued for information and the potential for media interest.
The State of New York and the-local law enforcement agency (LLEA) were informed.
