Forms Effective Dates EPF 06-004-01, Rev. 158 August 7, 2017 EPF 06-018-09, Rev. 9 August 7, 2017 EPF 06-018-12, Rev. 16 August 7, 2017 The attached summary of the changes to the implementing forms explains that these changes do not reduce the effectiveness of the WCGS RERP.
