NRC SPECIAL INSPECTION REPORT NOS. 50-335/99-09 AND50-389/99-09
Dear Mr. Plunkett:
This refers to the Training and Qualification Effectiveness inspection that was conducted for
cause during the week of March 15, 1999, at the St. Lucie facility. The purpose of the
inspection was to evaluate the effectiveness of your licensed operator training program and
your training program assessment.
The enclosed report presents the results of this inspection.
Your implementation of the Systems Approach to Training (SAT) was weak in several areas
and was a direct contributor to the poor pass rate on the initial licensing examination conducted
in December 1998. Your training program assessment
identified many of the issues caused by
your weak implementation of the SAT process.
Your initial corrective actions may have been
too narrowly focused and did not address all of the underlying weaknesses
in the
implementation of the SAT process.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and its
enclosure willbe placed in the NRC Public Document Room (PDR).
