At the conclusion of the exami-
nation, preliminary findings were discussed
with those
members of
your staff identified in the enclosed report.
During the examination visit, the examiners
made observatz.ons
concerning poor operator performance during a steam generator
tube rupture
(SGTR) event.
The operator actions
and your current
SGTR mitigation strategy
were discussed
with members of your
staff in a telephone
conversation
on May 20,
1991.
Their
proposals to address
our concerns
are detailed in.paragraph
4 of
this report and will be monitored during future exams.
Additionally, applications for the operators taking this
examination were not submitted in a timely manner.
We request
that future applications
be submitted in accordance
with guidance
in 10
CFR 55, Operator Licensing and
NUREG 1021, Operator
Licensing Examiner Standards.
In accordance
with 10
CFR 2.790 of the Commission's regulations,
a copy of this letter and the enclosures will be placed in the
NRC Public Document
Room.
