NOTICE OF VIOLATION Florida Power and Light Company St. Lucie 1
Docket No. 50-335 License No.
DPR 67 During the Nuclear Regulatory Commission (NRC) inspection conducted on December 6,
1987
- January 2,
- 1988, a violation of NRC requirements was identified.
The violation involved the failure to establish and/or implement procedures.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR Part 2, Appendix C (1987), the violation is listed below:
Unit 1 Technical Specifications (TS) 6.8. l.a, states that written procedures shall be established, implemented and maintained covering the applicable procedures recommended in Appendix "A" of Regulatory Guide 1.33, Revision 2, February 1978.
Regulatory Guide 1.33, Revision 2, February 1978, Appendix A, states in part, that procedures are required for startup, operation and shutdown of safety-related systems including electrical systems.
Contrary to the
- above, on December 21,
- 1987, procedure 1-0970020, operation of the 120V instrument AC system (Class 1E),
was not properly implemented in that while attempting to place the 1MD instrument inverter back into service following preventive mainte-
- nance, the inverter's output breaker was not closed, thereby causing loss of 120V instrument AC and a subsequent reactor trip.
This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Florida Power and Light Company is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including:
( 1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results
- achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.
Where good cause is
- shown, consideration will be given to extending the response time.
