NOTICE OF VIOLATION As u result of the inspection conducted on July 24 through October 22, 1987, and in accordance with the General Policy and Procedure for NRC Enforcement Actions, (10 CFR Part 2, Appendix C), the following violations were identified:

1.

Technical Specification (TS) 3.1.A.1 states in part, that "the minimum number of trip systems and instrument channels that must be operable for each position of the reactor mode switch shall be as given in Tab l e 3. 1. 1.

11 Contrary to the above, on August 18, 1987, Unit 21s reactor mode switch was in the 11 Run 11 position at a 94% power level with only one Average Power Range Monitor (APRM) Downscale/Intermediate Range Monitor (IRM) high high inoperable trip function (IRM-15/APRM-5) operable in RPS Channel B.

Having only one such trip function operable while in the 11 Run 11 mode, and not taking the appropriate actions required by TS Table 3.1.1 is in violation of TS Table 3.1.1, which requires a minimum of two operable APRM downscale/IRM high high inoperable trip functions per RPS Channel.

This condition existed for about 9 hours1.041667e-4 days <br />0.0025 hours <br />1.488095e-5 weeks <br />3.4245e-6 months <br />.

This is a Severity Level V violation (Supplement I).

Technical Specification 6.2.A.14 requires that "detailed written procedures including applicable checkoff lists covering items, 11 such as, "working hours of the Shift Engineer, Station Control Room Engineer, Shift Foreman, Nuclear Station Operator... shall be prepared, approved and adhered to.

11 10 CFR 50, Appendix B, Criterion V, as implemented by Commonwealth 'Edison Company, Topical Report CE-1-lA, 11Quality Assurance Program for Nuclear Generating Stations, 11 requires that activities affecting quality, such as Nuclear Station Operator working hours, shall be prescribed and accomplished by appropriate procedures.

This is a Severity Level V violation (Supplement I).

As described in Paragraph 3 of the report, the inspection showed that action had been taken to correct the identified violations and to prevent recurrence.

Consequently, no replies to these violations are required and we have no further questions regarding these matters.
