Enclosed is an eng;inee*ring evaluation report concerning several recent events involving deficient oper~tor attions following the failure of dual function (i.e., emergency core cooling system/containment isolation) valves.

The study found that light water ~eactors are generally equipped with a number of valves which perform both an emergency core cooling (or safety-related) function and a containment isolation function.

However, operating experience shows that the proper and conservative operator action for a f~ilure of one of these valves has not always been taken by the operating *staff in a manner which is fully consistent with the plant's technical specifications. It is suggested, therefore, that the Office of Inspection and Enforcement consider issuing an information notiCe which discusses the recent events involving deficient operator actions following dual function valve failures.

The study also suggests that the Office of Nuclear Reactor Regulation request all licensees f' p '&..

to examine their plant piping configurations and ensure that the dual function valves are also appropriately identified in the plant technical specifications table of containment isolation valves.
