DRESDEN INSPECTION REPORT 50-237/99003(DRP); 50-249/99003(DRP)

Dear Mr. Kingsley:

On January 10,.1999, through February 26, 1999, the NRC performed an inspection at your

Dresden Nuclear Power Station. The enclosed report presents the results of this inspection.

During the inspection period, a safety-conscious approach characterized the overall conduct of

activities at the Dresden station. Operators and station personnel performed well during the

conduct of the reactor shutdown and startup evolutions to support the planned refueling outage.

The conduct of most maintenance.activities and radiological control practices appeared to have

been performed correctly. However, a number of out-of-service errors occurred during the

outage. Further, initial corrective actions taken to address the errors were not effective in

stopping them.

While operators performed well during the conduct of routine and outage activities, we noted a

number of challenges that arose during the period. In five separate instances, actions by

station personnel resulted in the need for operators to enter unplanned Technical Specification

or Dresden Administrative Technical Requirement limiting conditions for operations. While the

limiting condition for operation action statements and time limits were not violated, each of thez

occurrences represented an u~necessary burden to the operators. We also noted procedural

concerns which challenged the operators. These included: unclear directions in an operator

surveillance that resulted in equalizing the drywell and the torus; a high pressure coolant

injection logic test performed on-line for the first time that required revision and delayed actions

in response to control panel alarms; and an instance where operators in the control room

performed a test on the reactor building to torus vacuum breakers without having the most

recent revision to the procedure.

Based on the results of this inspection, the NRC has determined that three violations ofNRC

requirements occurred. These violations are being treated as Non-Cited Violations (NCVs),

consistent with Appendix C of the Enforcement Policy. These NCVs are described in the

subject inspection report. If you contest the violations or severity levels of these NCVs, you
