NRC INSPECTION REPORT NO. 50-237/98009(DRP); 50-249/98009(DRP) AND

NOTICE OF VIOLATION

Dear Mr. Kingsley:

The NRC performed an inspection at the Dresden Nuclear Power Station from February 21

through April 3, 1998. The inspection included additional activities on April 30, 1998. The

purpose of the inspection was to assess routine performance. The enclosed report presents the

results of this inspection.

During this inspection period, a safety-conscious approach characterized the overall conduct of

acti_vities at the Dresden Station. The operators performed well during the conduct of t_he Unit 2

shutdown .to support the refueling outage and during the Unit 3 initial shutdowif activities fo

address scram discharge volume instrumentation concerns. However, the good performance

exhibited by operators during these evolutions was countered by a control rod mispositioning

error and by two fuel bundle errors that occurred during the Unit 2 core off-load.

The unavailability of the high pressure coolant injection system has been a continuing problem at

Dresden. During this inspection period, the Unit 3 high pressure coolant injection system was

again rendered inoperable due to an equipment failure. In this case, the specific equipment

failure was a repeat problem with the gland seal leakoff condenser level switch, indicating that

the station's past efforts to address the deficiency have not been completely*effective.

Based on the results of this inspection, the NRC determined that two violations of NRC

requirements occurred. The violations are cited in the enclosed Notice of Violation (Notice), and

the circumstances surrounding the violations are described in detail in the subject inspection

report. The first violation was of concern because it involved the failure to follow procedures

  • which represented a continuing trend of procedural and work instruction problems. The second

violation involved the failure to identify and correct a condition adverse to quality (an uncontrolled

stream of water in the drywell) and was of concern because station personnel in the area

seemed oblivious to the condition.

You are required to respond to this letter and should follow the instructions specified In the

enclosed Notice when preparing your response. The NRC will use your response, in part, to

determine whether further enforcement action is necessary to ensure compliance with regulatory

requirements. *
