NOTICE OF VIOLATION AND INSPECTION REPORT NO. 50-237/98003(DRP);
50-249/98003(DRP)
Dear Mr. Kingsley:
The NRC performed an inspection at the Dresden Nuclear Power Station from January 12
through .February 20, 1998, The purpose of .the inspection was to assess routine performance.
The enclosed report presents the results of that !nspection. * *
During this inspection period, a safety-conscious approach characterized the overall conduct of
activities at the Dresden Station. However, plant material condition and some maintenance
activities continued to challenge the operators. Inadequate written work instructions and
procedures impacted plant operations and maintenance. For example, an automatic reactor trip
occurred during a sur\\teillance test, and equipment was damaged due to installation of incorrect
parts during maintenance of an emergency diesel generator.
Some events caused by the material condition issues, such as the otcurrence of premature
hydrogen recombination in the off-ga.s *system, were repeat items, similar to those documented in
prior NRC inspection reports. The unavailability of the high pressure coolant injection (HPCI)
system has been a continuing prol:>lem at Dresden~ During this inspection period, both the
Unit 2 and 3 HPCI systems were again.rendered inoperable due to equipment.failures.
Based on the results of this inspection, the NRC determined that violations of NRC requirements
occurred. The violations are cited in the enclosed Notice of Violation (Notice), and the
circumstances surrounding the violations are described in detail in the subjec~ inspection report.
T_he violations all involved inadequate procedures or failure to follow procedures and represented
abontinuing trend of procedural and work instruction problems. In your response to the
violations for inadequate work instructions, you are requested to address the overall issue of
. preparing high-quality work instructions.
You are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part, to
determine whether. further enforcement action is necessary to ensure compliance with regulator)\\
. requirements.
