Dear Mr. Joyce:
SUBJECT:
EXAMINATION REPORT September 19, 1995 During the week of July 31, 1995, Mr. D. McNeil and others of this office administered initial license examinations to employees of your organization who applied for licenses to operate your Dresden Nuclear Power Station, Units 2 and 3.
At the conclusion of the examinations preliminary findings were discussed with those members of your staff identified in the enclosed report.
Four individuals not previously licensed at Dresden Units 2 and 3 were given Senior Reactor Operator (SRO) examinations. Three non-licensed operators at Dresden Unit 2 and 3 were given Reactor Operator (RO) examinations.
One RO candidate failed the written section of the examination.
One SRO candidate failed the written section of the examination.
One SRO candidate failed the written and dynamic simulator sections of the examination.
All other candidates passed all sections of their respective examination.
The candidates submitted for examination this year did not perform as well as candidates examined in previous years.
During the 1991, 1992 and 1993 license examinations, the candidates appeared to be strong, well prepared candidates.
The 1994 candidates displayed weaknesses on the written examination (3 written examination failures), but were well prepared for the Job Performance Measures (JPMs) and dynamic simulator scenario portions of the examination.
The 1995 written examination scores were lower than the 1994 written examinations (also 3 written examination failures).
But more significant than the written examination weaknesses, the 1995 candidates were weaker than the 1994 candidates in the operating (JPM and dynamic simulator) examination areas.
This trend indicates a significant decline in the license candidate training process.
The facility's failure rate over the last two years is 25%.
This is significantly above the national average of 5% and indicates the need for an internal evaluation of your training process to determine and correct root causes for the license candidates' poor performance during license examinations.
Please inform us of the findings of your internal evaluation within 30 days of the date of this letter.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and the enclosures will be placed in the NRC Public Document Room.
