Dear Mr. Reed:

  • This refers to the special inspection conducted by W. G. Rogers,

D. E. Hills, and M. S. Peck of this office on September 1 through

November 8, 1991.

The inspection included a review of ~ctivities

authorized for your Dresden facility.

At the conclusion of the inspection,

the findings were discussed with those members of your staff identifi~d ih

the ehclosed report.

Areas examined during the inspection are identified in the report.

Specifically, the circumstances surrounding ari increase in torus water

temperature beyond Technical Specification limits, a 2800 gallon

contaminated water spill from the control rod drive system, and the

scramming of a control rod out of sequenc~ during routi~e testing

activities were examined.

Based on the result f this inspection, four apparent violations were

identified and are eing c6nsidered for escalated enforcement action in

accordance with the "General Statement of Policy and Procedure for NRC

  • Enforcement Action

11 (Enforcement Policy), 10 CFR Part 2, Appendix C

(1991).

The appa ent violations involved failures to follow administrative

and operating pr cedures during the conduct of operations, as well as

  • inadequate adm* istrative proced~res to ade4uately control activities ..

Thes~ apparent violations in conjunction with other recent e~ents that

appear to have similar causes are being reviewed by NRC management.

Accordingly, no Notice of Violation is p~esently being issued for these

inspection findings.

In addition, please be advised that the number and

characterization of apparent v~olations described i.n the enclosed

inspection report may change as a result of further NRC

revi~w.

You will

be advised by separate correspondence of the results of our deliberations

on this matter.

No response regarding these apparent violations is

required at thi~ time.

In accordance with 10 CFR 2.790, of the Commission's regulations, a copy of

this letter and the enclosure(s) will be placed in the NRC Public Document

Room.
