SPECIAL NRC INSPECTION (REPORT NO. 50-249/91032(DRS))

Dear Mr. Reed:

This refers to the speqial safety inspection conducted by

Peggy R. Rescheske from this off ice on October 21 through

November 12, 1991.

The inspection included a review* of

activities authorized by NRC Operating License No. DPR-25 for

the Dresden Nuclear Power Station, Unit 3.

A discussion of our

.findings was held with Mr. L. Gerner. and other meinbers of your

staff at the conclusion of the inspection.

The inspection focused on the October 18-19; 1991, fuel handling

event in.which two fuel bundle bail handles were damaged in the

spent fuel pool.

The enclosed copy of our inspection repqrt

identifies the areas examined during the inspection.

Within

these areas, the inspection consisted of a selective examination

of procedures and representative records, observations, and

interviews. with personnel.

Based on the results of this inspection, one apparent violation .

was identified.

The apparent violation involved the failure to

adequately implement the station procedure for fuel movement

within the spent fuel pool, as required by Dresden Technical

Specification 6.2.A.

The inattention to detail during fuel

movement resulted in damage to two fuel assembly bail handles.

Subsequently, three additional fuel assemblies were moved with

the damaged mast. *operations management (other.than the fuel

handling supervisor) was not informed of the personnel error, or

damage to the fuel assemblies and* mast, until the next shift.

Expectations that work should be stopped in the event of

personnel error or unusual occurrence had not been clearly

communicated to the individuals involved in the event.

Further,

a lack of training and a lack of procedures regarding response.to

unusual incidents were evident.

On October 22, 1991, as a result

of this event, Region III issued a Confirmatory Action Letter

(CAL-RIII-91-014) regarding resumption of fuel movement at

Dresden.

The apparent violation discussed above, along with three other

apparent violations involving recent operational events (NRC

Inspection Reports No. 237/91035(DRP): No. 249/91038(DRP)), are

being considered for escalated enforcement action in accordance

with the "General*Statement of Policy and Procedure for NRC

Enforcement Actions" (Enforcement Policy), 10 CFR Part 2,

Appendix C (1991).

No Notice of Violation is being issued at

this time for these inspection findings.

Please be advised that

the number and characterization of the apparent violations may

change as*a result of further NRC review.

An enforcement conference to discuss the apparent violations has

been scheduled for December 10, 1991.

The purposes of this

conference are to discuss the apparent violations, the causes,

and safety significance: to provide you with the opportunity to

point out any errors in our inspection report: to provide an

opportunity for you to present your proposed corrective actions: .

. and to discuss any other information that will help us determine

the appropriate enforcement action in accordance with the

Enforcement Policy.

You will be advised by separate

correspondence of the results of our deliberations on this

matter.

No response regarding the apparent violation described

in the enclosed inspection report is required at this time.

In accordance with 10 CFR 2.790 of the Commission's regulations,

a copy of this letter and .its enclosure will be placed.in the NRC

Public Document Room.
