NRC INTEGRATED INSPECTION REPORT 50-387/98-07, 50-388/98-07

AND NOTICE OF VIOLATION

Dear Mr. Byram:

On August 31, 1998, the NRC completed an inspection at your Susquehanna

Steam

Electric Station (SSES) Unit 15. 2 reactor facilities. The inspections covered routine

activities by the resident inspectors,

and announced inspections by Region

I Operator

Licensing Specialists,

Region

I Radiation Specialists,

and a Region

I Emergency

Preparedness

Specialist.

The enclosed report presents the results of these inspections.

The inspectors discussed the findings of these inspections with Mr. R. Saunders,

Vice

President Nuclear Operations, at an exit meeting at the completion of the inspection period.

During the 6-week period of inspection, your conduct of activities was characterized

by

safe operation and generally conservative decision making.

However, we concluded that

reactivity controls during the Susquehanna

Unit-2 plant startups on June 26, 1998 and

July 2, 1998 were not adequate.

Based on the results of this inspection, the NRC has determined that a violation of NRC

requirements occurred.

The violation is cited in the enclosed Notice of Violation (Notice)

and the circumstances

surrounding it are described in detail in the subject inspection

report.

This violation is of concern since the July 2, 1998 reactivity control event on Unit-.

2 was preventable if your operating staff had correctly used your condition reporting

system following a similar reactivity control event on June 26, 1998. This Severity Level

IV violation could meet the criteria of Section VII.B.1 of the Enforcement Policy and be

considered

a Non-Cited Violation. However, because of the importance of reactivity

management

and the importance of an effective corrective action system, the NRC has

determined that the violation of NRC requirement 10 CFR.50 Appendix B Criterion XVI

"Corrective Action" should be cited.

The NRC has concluded that information regarding the reason for the violation, the

corrective actions taken and planned to correct the violation and prevent recurrence

is

already adequately addressed

in this inspection report.

Therefore, you are not required to

respond to this letter, unless the description therein does not accurately reflect your

corrective actions or your position.

In that case, or if you choose to provide additional

information, you should follow the instructions specified in the enclosed'Notice.

and

radioactive material transportation programs.

Performance

in these areas was concluded

to be effective.
