COMBINED INSPECTION
REPORT
NOS. 50-387/94-23
AND 50-388/94-24
Dear Mr. Byram:
This letter transmits
the report of the announced
safety inspection
conducted
by Mr. Aniello L. Della Greca of this office for the period of
October 24-28,
1994, at the Susquehanna
Steam Electric Station
(SSES)
in
Berwick, Pennsylvania.
The inspection
included
a review of activities
authorized for your Susquehanna
facility.
At the conclusion of the
inspection,
the findings were discussed
with Mr. Gene Stanley.
The inspection
was directed toward areas
important to public health
and
safety.
Areas
examined during this inspection
are described
in the
NRC
inspection report enclosed with this letter and included the actions taken
by
Power
and Light Company in response
to
and
Supplement
1,
"Loss of Fill-Oil in Transmitters
Manufactured
by Rosemount."
Within this scope,
the inspection consisted of selective
examinations of design
documents,
procedures
and representative
records,
interviews with personnel,
and observations
by the inspector.
The inspector
determined that,
based
on the safety function of the Rosemount
transmitters
in use at
SSES,
your actions to address
the loss of fill-oil
concern
were generally acceptable.
Adequate training had been provided to the
technical
and operating staff to understand
the issues
associated
with the
transmitter failure mode
and to recognize its symptoms.
Also, the scope of
monitoring was well beyond the requirements
of the bulletin supplement.
However, the inspector also identified several
weaknesses
that require your
attention.
In particular, the authorities
and responsibilities
had not been
clearly defined in a plant procedure.
Because of this, the calibration
results
were not always distributed to the engineering staff and there
was
no
evidence that the trended data
was evaluated fully and in a timely manner.
The inspector's
observations
resulted in two unresolved
items pertaining to
the formalization of the program,
and to the evaluation of several
transmitters with abnormal
trends.
