NRC INTEGRATED INSPECTION REPORT 50-387/97-02,
50-388/97-02
Dear Mr. Byram:
This refers to the inspections conducted
between February 15, 1997 and April 7, 1997, at
the Susquehanna
Steam Electric Station
1 5 2 reactor facilities. The inspections covered
routine activities by the resident inspectors
and announced
inspections by a Region
I health
physics specialist inspector.
The enclosed report presents the results of these inspections.
Also enclosed
is a copy of the presentation
material you provided at a pre-decisional
enforcement conference
held in the Region
I Office, King of Prussia,
PA on March 21,
1997.
That conference
dealt with several apparent violations associated
with the
inoperability of an emergency diesel generator
in July, 1996.
Overall, the conduct of operations at the Susquehanna
facility during this period.was
characterized
by safe operation and conservative
decision making.
In two instances
control room operators responded
well to plant transients.
In one instance you were very
successful
in the identification and resolution of a technical issue involving response
time
testing of safety related instrumentation.
During this period, two separate
maintenance
related activities and one engineering related
activity resulted in events that could have resulted in serious personnel injury. One
additional maintenance
related activity resulted in an operator challenge and plant transient.
Even though there were no violations of NRC requirements,
each of these events could
have had significant consequences,
It was noted that Susquehanna
plant management
responded
well to each event from a corrective action and investigation standpoint.
In the area of plant support, we found that you continued to maintain an effective
radiological controls program.
This was evidenced
by your extensive planning and
effective implementation of radiological controls for outage work.
You are requested to respond to this letter within 45 days, concerning the non-safety
related maintenance
induced incidents discussed
above.
Based on your investigations,
please indicate how you intend to control these types of maintenance/engineering
related
activities in order to prevent operator challenges,
plant transients,
and/or increased
potential for personnel
injury. Also, please discuss any precursor events and why

events that occurred during this inspection report period.
