Dear Mr. Byram:
On June
10,
1996, the
NRC completed
an inspection at your Susquehanna
Steam
Electric Station
1 L 2 reactor facility.
The enclosed report presents
the
results of that inspection.
During the six week period covered
by this inspection,
your staff's conduct of
activities at the Susquehanna facility was characterized
by safe operation
and
conservative
decision making.
However, in one instance
the timeliness
and
adequacy of your engineering staff's response
to a potentially degraded
condition did not reflect their usual
high level of performance'.
This issue
involved a turbine bypass
valve stroke test in which the test results for the
time-to-open criteria were satisfactory,
but the valve did not cycle smoothly,
and
as
a result operability of the fast acting solenoid could not be
confirmed.
The fast acting solenoid
must
be operable for the valve to perform
its function.
Your staff determined
the valve to be operable
based
on it
passing
the time-to-open criteria without addressing its irregular motion and
operability of the fast acting solenoid.
Of concern
was that it took four
days to resolve this issue, particularly since the operability of this valve
is important
as it has
a significant role in mitigation of certain types of
