Dear Nr. Byram:
On March 18,
1996,
the
NRC completed
an inspection at your Susquehanna
Steam
Electric Station
(SSES),
Salem Township,
The enclosed report
presents
the results of that inspection.
During the six week period covered
by the inspection period,
conduct of
activities at the Susquehanna
facilities was generally characterized
by safety
conscious
operation
and maintenance
practices.
The licensed operator
requalification examinations effectively evaluated
operator ability to safely
operate
the plant and operators
were held to high standards
of performance.
The Inprocess
Corrected
Errors trending
by guality Control personnel
was
considered
a good initiative.
A violation of NRC requirements
was identified, involving a secondary
containment isolation damper that did not fully close during
a technical
specification required surveillance test.
The operators
failed to promptly
identify the degraded
damper condition
and take appropriate
action.
Work
records
show that indication of the degraded
was first observed
four
months earlier;
however,
operators failed to recognize its significance or
engage
the corrective action process.
This violation is cited in the enclosed
Notice of Violation, and the
circumstances
surrounding
the violation are described
in detail in the
enclosed report.
Please
note that you are required to respond to this letter
and should follow the instructions specified in the enclosed
Notice when
preparing your response.
The
NRC will use your response,
in part, to
determine whether further enforcement
action is necessary
to ensure
compliance
with regulatory requirements.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter and its enclosures will be placed in the
NRC Public Document
Room.
