Dear Mr. Byram:
An announced
safety inspection of the radiological controls
program at your
facility was conducted
on site
by Mr. Randolph
Ragland
on September
12
September
16,
1994, of activities authorized
by
NRC License
Nos.
and
At the conclusion of the inspection
on September
16,
1994, the
findings were discussed
by Mr. Ragland with Mr.
G, Kuczynski
and other
members
of your staff.
Areas
examined during this inspection
are important to health
and safety
and
are described
in the
NRC Region I Inspection
Report that is enclosed with this
letter.
The inspection
was conducted
to review the implementation of the
revised
10 CFR 20 regulations
including use of new radiation
dose limits,
definitions,
and respirator philosophy.
Special
emphasis
was placed
on
procedures,
training,
and policies associated
with control of high radiation
areas
(HRAs)
and very high radiation areas
(VHRAs), planned
special
exposures
(PSEs),
declared
pregnant
women
(DPW), control of embryo/fetus
dose,
and
reviews for maintaining total effective dose equivalent
(TEDE)
as low as
reasonably
achievable
(ALARA).
Within these
areas,
the inspection consisted
of selective
examinations
of procedures
and representative
records,
interviews
with personnel,
and observations
by the inspector.
The overall
implementation of the revised
10 CFR 20 regulations
was found to
be very good.
Training lesson
plans were comprehensive
and complete,
implementing procedures
included appropriate
changes,
and individual knowledge
of the changes
to the regulations
was good.
Procedural
controls, training,
and policies for
HRAs and
DPW, embryo/fetus
dose,
and maintaining total
effective dose equivalent
ALARA were found to be very good.
Our inspector
reviewed
an August 30,
1994, incident that involved significant
potential for
an unplanned
exposure to several
workers.
A system evolution
that would have significantly increased
radiation levels in the Unit 2
Condenser
Demineralizer
Resin Regeneration
(CDRR)
Room was initiated without
prior 'verification that the
room was unoccupied.
Fortunately,
the Nuclear
Plant Operator controlling the evolution noted the presence
of the individuals
by viewing the
CDRR
Room on
a remote television monitor
and warned
them
immediately to leave the area.
They did so
and thus did not receive
a
significant radiation exposure.
While no regulatory requirements
appear to
have
been violated,
and corrective actions
had
been initiated at the time of

the inspection,
the lack of procedural
controls to prevent
such
an occurrence
is of concern to us.
If not already
completed
as part of your corrective
action,
a thorough review to assure
that similar procedural
weaknesses
do not
exist elsewhere
at the station is needed.
Please
provide us; within 30 days
of the date of this letter,
a summary of the actions
completed
and planned to
address this safety concern.
No other safety concerns
and
no violations of NRC regulatory requirements
were
identified.
The response
requested
to this letter is not subject to the
clearance
procedures
of the Office of Hanagement
and Budget
as required
by the
Paperwork Reduction Act of 1980,
