NRC REGION I COMBINED INSPECTION 50-387/93-19; 50-388/93-19

The enclosed report documents

a resident safety inspection conducted by Messrs. G. S.

Barber and D. J. Mannai, and others from October 5, 1993 through November 15, 1993, at

the Susquehanna

Steam Electric Station (SSES), Salem Township, Pennsylvania.

The

inspection covered plant operations,

surveillance, and maintenance for both units. At the

conclusion of the inspection, Mr. G. S. Barber, Senior Resident Inspector, summarized the

inspection findings with Mr. H. G. Stanley and other members of your staff.

During the current refueling outage, we noted that a control rod withdrew independent of a

demand signal.

Our inspector reviewed your operational history and noted that there were at

least 16 previous occurrences

when a control rod moved independent of a demand signal.

Seven of these were reportedly due to malfunctioning transponder cards.

We are concerned

with the frequency of these occurrences,

your apparent treatment of these events as isolated

cases,

and the consequent failure to recognize, evaluate,

and address

trends in performance

that could potentially affect safety and quality. Accordingly, we willcontinue to monitor

your current efforts to understand

and resolve the root cause conditions that have so far

prevailed.

A management

meeting on this topic may be required as more information is

developed.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and

its enclosures willbe placed in the NRC Public Document Room.
