SUBJECT:
CLOSEOUT OF CONFIRMATORY ACTION LELTER 1-93-18 Confirmatory Action Letter (CAL) 1-93-18 was provided to you on October 29, 1993, to confirm your actions and agreement relative to accommodating the requirements of an Augmented Inspection Team (AIT) that was dispatched to the Susquehanna Stea'm Electric Station on that date to review the events and circumstances involving a series of fuel handling equipment problems that occurred during the current Unit 1 outage.
This agreement involved curtailing further refueling activities at Susquehanna Unit 1 until the AIT acquired all the information needed for their assessment and was satisfied that adequate corrective measures have or would be taken.
Associated actions included assembling and making available for review, all documentation, equipment, and components associated with the fuel. handling activities; and making personnel available to the AITfor interviews, as necessary.
Further, you agreed to keep the AITleader cognizant of the operation, testing, maintenance, repair, and surveillance of fuel handling equipment; and assure his agreement prior to any resumption of refueling activities.
On November 9, 1993, the AIT completed its on-site activities.
We have reviewed your responses to the CAL (correspondence dated November 4, November 5, and November 18, 1993) and have subsequently confirmed that you have successfully completed all of the commitments contained in the CAL.
On November 18, 1993, a management meeting was held at our Region I office.
At that meeting your staff presented their assessment, root cause determination, and corrective measures relative to fuel handling equipment events; and described actions taken in preparation for resumption offuel handling activities. On November 19, 1993, you were informed by Mr. John R. White of this office, that we agree to resumption of refueling activities, based on the information you presented in the November 18, 1993 management
- meeting, and our own independent review and assessment of your revised procedures, personnel training and qualification, and management oversight and control improvements.
Accordingly, we consider that Confirmatory Action Letter 1-93-18 is closed.
