NRC REGION I COMBINED INSPECTION 50-387/93-13; 50-388/93-13

The enclosed report documents

a resident safety inspection conducted by Messrs.

G. S.

Barber and D. J. Mannai, and others from August 18, 1993 through October 4, 1993, at the

Susquehanna

Steam Electric Station (SSES), Salem Township, Pennsylvania.

The inspection

covered plant operations,

surveillance, and maintenance for both units. At the conclusion of

the inspection, Mr. G. S. Barber, Senior Resident Inspector, summarized

the inspection

findings with Mr. H. G. Stanley and other members of your staff.

During the period, a safety related Circle Seal Solenoid Operated Valve (SOV) failure caused

an unplanned Engineered

Safety Feature (ESF) actuation.

Your staff initiated a detailed

investigation which determined breakdown of the coil wire insulation caused the SOV failure.

This course of action was considered prudent given the industry history of SOV failures.

SOV failures are of particular concern due to the potential for common-mode failures or

degradations of safety related systems.

We understand

that you willbe providing the failure

analysis details and a history of Circle Seal SOV failures at Susquehanna

in a supplemental

Licensee Event Report (LER).

As a result of our review of a previous open item we noted your positive corrective actions

for area contaminations

that occurred during spent fuel cooling demineralizer backwashes

and

precoats.

As a result, we have chosen not to cite a procedure violation that resulted in one

of the contaminations.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and

its enclosures will be placed in the NRC Public Document Room.
