NRC REGION I COMBINED INSPECTION 50-387/92-29; 50-388/92-29
The enclosed report documents
a resident safety inspection conducted by Messrs.
G. S.
Barber and D. J. Mannai, and others from November 10, 1992 through December 31, 1992,
at the Susquehanna
Steam Electric Station (SSES), Salem Township, Pennsylvania.
The
inspection covered plant operations,
surveillance, and maintenance for both units. At the
conclusion of the inspection, Mr. G. S. Barber, Senior Resident Inspector, summarized the
inspection findings with Mr. H. G. Stanley and other members of your staff.
During the inspection period your staff identified radiological contamination of the "B"
Auxiliary Boiler which resulted in a minor unmonitored, uncontrolled release of noble gases
to the environment,
Prompt action was taken to identify and assess
the radiological
consequences
of the event as well as secure the source of contamination.
This event marked
- the third auxiliary boiler contamination from essentially the same cause since May 1988.
We
are particularly concerned with the lack of lasting corrective action regarding this matter.
Although this most recent event had minimal safety significance, we are interested in your
actions and plans to prevent recurrence.
We are interested in ensuring that you provide
lasting corrective action for this deficiency.
Therefore, we ask that you respond by
providing your actions to prevent recurrence within 45 days of the receipt of this letter.
Your cooperation with us in this matter is appreciated.
