NRC REGION I COMBINED INSPECTION 50-387/92-16; 50-388/92-16

The enclosed report documents

a resident safety inspection conducted by Messrs.

G. S. Barber and D. J. Mannai, and others from June 1, 1992 through July 13, 1992, at the

Susquehanna

Steam Electric Station (SSES), Salem Township, Pennsylvania.

The inspection

covered plant operations, surveillance, and maintenance for both units. At the conclusion of

the -inspection, Mr. G..S. Barber, Senior Resident Inspector, summarized the inspection

findings with Mr. H. G. Stanley and other members of your staff.

During this inspection, we focused particular attention to the overheating of the Unit 1 "B"

offgas guard bed.

We recognized that, as a result of this occurrence, your staff initiated an

aggressive investigation effort to identify root causes and corrective actions.

Your efforts

were comprehensive

and thorough.

As a result of your Event Review Team's (ERT) investigation, several root causes and actions

to prevent recurrence were identified.

Among these was an inadequacy pertaining to the

physical orientation of the drain traps that were designed to minimize liquid intrusion into the

offgas guard bed.

In addition to this orientation error, your staff also found that the elevation

differences of the traps would have precluded proper draining.

We are particularly concerned

with this finding since you did not identify this installation error during your investigation of

a past similar guard bed overheating that occurred on April 11, 1987.

However, we noted

that your current root cause identification program is significantly better than the 1987

program and that you promptly identified and corrected the drain trap installation errors.

We

also noted you wilLreview the drain trap installation configurations for other plant systems.

Based on the positive actions you have taken, a non-cited violation is appropriate in this case.
