The enclosed report documents a resident safety inspection conducted by Messrs. G. S. Barber
and
D. J.
Mannai,
and others
from February
11,
1992 through April 18,
1992,
at the
Susquehanna
Steam Electric Station (SSES), Salem Township, Pennsylvania.
The inspection
covered plant operations, surveillance, and maintenance for both units. At the conclusion of the
inspection, Mr. G. S. Barber, Senior Resident Inspector, summarized the inspection findings with
Mr. H. G. Stanley and other members of your staff.
Based
on results of this inspection, it appears
that one of your activities was not in full
compliance with NRC regulations.
Since 1984, there have been 34 separate
instances
where
Reactor Protection System Electrical Protection Assembly breakers were found tripped or were
known to have tripped.
The Electrical Protection Assembly breaker trips were attributable to
unknown causes or to problems with logic cards and related components.
The frequency of these
trips has increased within the past year even though action has been taken to preclude repetition.
Corrective actions to date have been ineffective.
Accordingly, this matter is identified as a
violation of 10 CFR 50, Appendix B, Criterion XVI, Corrective Action and the Susquehanna
Quality Assurance
Program,
relative to failure to take prompt corrective action to preclude
repetition.
This issue is set forth in the Notice of Violation enclosed
as Appendix A.
