Subject: NRC Region I Combined Inspection 50-387/92-02; 50-388/92-02 and

NOTICE OF VIOLATION

The enclosed report documents

a safety. inspection conducted by our senior resident inspector

from January

1, 1992 - February 10, 1992, at the Susquehanna

Steam Electric Station

(SSES), Salem Township, Pennsylvania.

The inspection covered your conduct of licensed

activities for both units. At the conclusion of the inspection, Mr. 6, S. Barber, Senior

Resident Inspector, summarized the inspection findings with Mr. T. C. Dalpiaz and other

members of your staff.

During this inspection, we focused particular attention to the hydrogen gas detonation event

of January

18, 1992, involving repair activities to the common off-gas recombiner system.

We recognized that, as a result of this occurrence,

your staff initiated an aggressive

investigation effort to identify root causes

and corrective actions.

We are satisfied that your

effort was sufficiently comprehensive

and thorough.

As a result of your investigation of this self-disclosing event, your staff identified several

weaknesses.'elative

to these findings, we are particularly concerned about (1) the apparent

ineffective incorporation of notifications, information, and communications of previous

industry events involving combustible gas hazards associated

with off-gas systems into

pertinent plant procedures

and practices;

(2) the failure to adequately track the status of the

1989 repair activity involving the globe valve (HV-16907) that was used to isolate the

common recombiner system in this instance;

and (3) the failure to adequately investigate the

cause for the unusual off-gas instrument indications on January

16. 1992, which were

apparently caused by a small hydrogen detonation at that time.

Relative to these matters, we

are continuing to review your process for handling generic communications and notifications

of previous industry events; and have identified the remaining items as apparent violations as

described in the enclosed Notice of Violation.
