NRC INTEGRATED INSPECTION REPORT 50-387/99-06 and 50-388/99-06
Dear Mr. Byram:
On July 19, 1999, the NRC completed an inspection at your Susquehanna
Steam Electric
Station (SSES) Unit 18 2 reactor facilities. The enclosed report documents the results of the
inspection of routine activities by the resident inspectors and an announced inspection of your
security program by Region
I security specialists.
The inspectors discussed the findings of
these inspections with Mr. R. Saunders, Vice President Nuclear Operations, and others of your
staff, at an exit meeting at the completion'of the inspection period.
During this period, we observed that your staff safely operated the facilityduring normal
operations, unplanned power reductions, and two automatic reactor shutdowns.
In addition, we
reviewed several areas in your security program and found them acceptable.
We noted that equipment failures caused the unplanned power reductions and the two
automatic reactor shutdowns during this period. These failures resulted, in part, from
deficiencies in past maintenance practices or narrowly focused corrective actions for previous
problems. We understand that your staff has recently formed an equipment reliability review
team to recommend changes to improve equipment performance.
Based on the results of this inspection, the NRC has determined that one Severity Level IV
violation of NRC requirements occurred.
This violation is being treated as a Non-Cited Violation
(NCV), consistent with Appendix C of the Enforcement Policy. This NCV is described in the
subject inspection report and involved a failure to make notifications to the NRC within the
required time. We noted that this violation occurred, in part, because of performance errors by
members of your staff. Ifyou contest the violation or severity level of this NCV, you should
provide a response within 30 days of the date of this inspection report, with the basis for your
denial, to the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington DC
20555-0001; with a copies to the Regional Administrator, Region I; the Director, Office of
Enforcement, United States Nuclear Regulatory Commission, Washington, DC 20555-0001; and
the NRC Resident Inspector at the Susquehanna
Steam Electric Station.
