APPENDIX A NOTICE OF VIOLATION Pennsylvania Power 8 Light Company Susquehanna Unit 1

Docket No. 50-387 License No ~ NPF-14 During an NRC inspection conducted on August 16, 1987 - September 30,

1987, one violation of NRC requirements was identified.

In accordance with the "General Statement of Policy and Procedures for NRC Enforcement Actions",

10 CFR Part 2,

Appendix C

(NRC Enforcement Policy), the violation is listed below:

Technical Specification 3.6.5. 1 requires that secondary containment integrity be maintained while in Operational Condition 1.

In accordance with Technical Specification 1.37 and 4.6'. l.b.3, included in the requirement for secondary containment integrity is that all secondary containment penetrations required to be closed during accident conditions, including penetrations between

zones, are either closed or capable or being closed by an autoofatic isolation system.

Contrary to the

above, during the period August 31 September 4,
1987, while in Operational Condition 1,

secondary containment penetrations (manual dampers XD-17513 and XD-17514) between Zones I and III, required to be closed during accident conditions, were not closed or capable of being closed by an automatic isolation system.

This is a Severity Level IV Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Pennsylvania Power and Light is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including:

( 1) the reason for the violation if admitted; (2) the corrective steps which have been taken and the results achieved; (3) the corrective steps which will be taken to avoid further violations; and (4) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending this response time.
