NOTICE OF VIOLATION Pennsylvania Power and Light Company Susquehanna Steam Electric Station, Unit 1

NPF-14 EA 86-106 During an NRC inspection conducted on March 15-April 15,

1986, NRC inspectors reviewed the circumstances associated with a violation of a technical speci-fication limiting condition of operation (LCO).

The violation, which was identified by the licensee, involved the inoperability of two of four scram discharge volume high water level transmitters for an amount of time (approx-imately one year) in excess of the LCO action statement.

The transmitters, which provide input signals to the Reactor Protection

System, were inoperable in that their isolation valves were closed.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions",

10 CFR Part 2, Appendix C (1985) (Enforcement Policy), the violation is set forth below:

Technical Specification Limiting Condition for Operation (LCO) 3.3. 1 and Table 3.3. 1-2 requires that whenever the reactor is in the power opera-tions or startup condition, (or in the refueling condition with any control rod withdrawn, other than control rods removed per Specification 3.9. 10. 1 or 3.9. 10.2),

two Reactor Protection System instrumentation channels per trip system shall be operable for the scram discharge volume high water level transmitters, or the affected trip system( s) must be placed in the tripped condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.

Contrary to the above, between June 8, 1985 and February 15, 1986, while the reactor was in a power operations condition except for three short forced outages, only one Reactor Protection System instrumentation channel per trip system was operable for the scram discharge volume high water level transmitters, and during that time, the trip system was not placed in the tripped condition.

The other transmitters, LT-C12-1N016C and D, were inoperable because of locked isolation valves.

This is a Severity Level IV Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Pennsylvania Power and Light Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or ex-planation in reply, including:

( 1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending this re-sponse time.
