APPENDIX A NOTICE OF YIOLATION Pennsylvania Power 8 Light Company Docket No. 50-387 This refers to the inspection conducted by a representative of the Region I (Philadelphia) office at the Susquehanna Steam Electric Station, Salem Township, Pennsylvania, of activities authorized by NRC License No. CPPR-101.
During this inspection conducted on June 10-.13,
- 1980, and June 16, 1980, the following apparent item of noncompliance was identified.
10 CFR 50, Appendix B, Criterion V, states, in part, "Activities affecting quality shall be prescribed by documented instructions, procedures,...and shall be accomplished in accordance with these instructions, procedures, or drawings...."
The Susquehanna PSAR, Section D.2.5 states, in part, "Activities affecting quality shall be prescribed by documented instructions',
procedures or drawings appropriate to the circumstances."
Section 6.3A of Nuclear Energy Services, Inc. procedure 80A9068, Revision 1, states, in part, "For Level I or Level II certification an employee must satisfy the following requirements 1.
Experience as specified in Table l.
2.
Training in a course...The length of time required for each course is found in Table 2 in correlation with the employee's education."
Table 2 of Nuclear Energy Services, Inc. document 80A9068 specifies that a
high school graduate requires forty hours training for Level I certification and forty hours additional training is required for certification to Level II in the ultrasonic method.
Contrary to the above, on June 12, 1980, residual heat removal system weld number GBB-104-2-2A was examined ultrasonica'lly by a Nuclear Energy Services, Inc. employee, certified to UT Level II, who did not receive eighty hour s of training.
This is a deficiency.
