BACKGROUND

A.

Reason for Investi ation

The Atomic Safety and Licensing Board (ASLB) held a pre-hearing

conference

in Milkes Barre, Pennsylvania

on January 29-31,

1979, with respect to,

Pennsylvania

Power and Light Company's

(PP8 L) application for an operating

license for Susquehanna

Units 1 and 2.

During the conference,

a limited

appea. ance statement

by Mary Kelchner

Creasy contained allegations

regarding

lost drawings,

drunken inspectors

and supervisors,

and qualifications of

guality Control inspectors.

The ASLB requested

that the Nuclear Regulatory

Commission review these

matters.

An investigation regarding the allegations

contained in the statement

was

initiated at the Susquehanna

Units 1 and

2 site by the

NRC, Region I on

February 21,

1979.

as missing from stick files

and drawing sepias

and microfilm cards that were not on file

in the print room.

found no information or evidence. that

had been irretrievably lost.

Conclusions

There

was

no substantiation

found for any of the allegations

and

no items

of noncompliance with regulatory requirements

were identified during the

investigation.

III.

OETAILS

A.

Introduction

This investigation was'nitiated

as

a result of the contents of a limited

.appearance

statement. made

by an individual during a pre-hearing

conference

conducted

by the ASLB on January

29-31,

1979, at Wilkes-Barre,

Pennsylvania

with respect to an application from PP8L for an op'crating license for

Susquehanna

Units 1 and 2.

The NRC-Region I contacted

the individual by telephone

on February 1, 1979,

to schedule

an interview for additional information regarding the statement.

During subsequent

telephone

discussions

on February 5, 1979,

between the

NRC-Region I, the individual, and the attorney representina

the individual,

it was established

that an interview would depend

on the individual being

provided

a copy of papers

and records

by Bechtel regarding

her period of

employment.

Further efforts to interview the individual or obtain information

from her for the purpose of aiding this investigation were unsuccessful

as of

the date of this report.

"I am a former employee of Bechtel Corporation.

I was in their

employ for nine months.

While I was with Bechtel, in my opinion I

saw incompetence

at all levels encountered

from lost drawings,

not

meaning blueprints,

drawings, to drunken inspectors

and supervisors.

The qualifications of the

gC inspectors

is also questionable

in my

mind;

a few people I know having

no background in the nuclear industry

or construction industry were given jobs beyond their expertise."

in the Small Piping Engineering group.

The investigators

found that an

NRC inspection of Susquehanna

was conducted

from October

22 to 24, 1975.~

An item of noncom-

pliance

was identified during this inspection regarding the

failure to control voided production drawings in the construc-

tion areas of the Reactor Building and Primary Containment to

preclude their inadvertent

use

by the construction crafts.

was forwarded to PAL

by letter dated

November 7, 1975.

The corrective

measures

regarding the item were described

in a letter to the

NRC from

PP&L dated

December 5,

1975.

During an inspection

conducted

by

the

NRC on March 30 and 31,

1976 the corrective measures

were

verified by the inspector

and the matter was considered to be

resolved.~

The investigators

did not observe

any other items of noncompliance

regarding the control of'rawings in reviewing the inspection

history during the period from October 24,

1975 to July 22,

1976.

The investigators

examined the following items regarding this

The investigators

noted that the audits were conducted

and discrepancies

identified during the audits were

corrected in a timely manner.

issued

to field location stick files; drawings

on stick files

that did not have Design

representatives

and;

several

individuals that were working at the Susquehanna

Units 1

and

2 site during the period from October 24,

1975 to July 22,

1976.

None of these individuals could recall

any instances of

an inspector or supervisor

being drunk on the site or of any

.instances

of observing

an inspector or supervisor performing

'heir assigned duties'ith. indications of being adversely

affected by.,alchohol.

Due to the general

nature of the allegation,

the investigators

did not request to examine

any individual personnel

records.

NRC Findin s

The

NRC investigation found no information or evidence that

there were drunken inspectors

or supervisors at the site during

the period from October 24,

1975 to July 22, 1976.

NRC Investigation

The investigators

interpreted this allegation to be related to

the guality Control inspectors

employed

by Bechtel during the

employment period of Mary Kelchner,

now by marriage

Mary Kelchner

Creasy.

Based

on Bechtel guality Control records the investigators

determined that there were

a total of 85 gC Engineers

employed

during the period from October 24,

1975 to July 22, 1976.

The investigators

examined the qualification records of each

of'he

85 gC individuals to verify that their qualification had

been

documented

in accordance

with Bechtel Field Inspection

Manual Procedure

G-8, Revision 0, dated

June 3, 1974, titled,

"gualifications, Evaluation,

Examina'tion, Training and Certifi-

cation of Construction

Department

equality Control Personnel"

and Project Special

Provisions Notice SF/PSP G-8.1, Revision 0

dated August 10,

1976 related to Procedure

G-8.

The following administrative deficiencies

were observed in the

qual ificati on records of QC personnel:

(1)

The Performance

Oemonstration

Record

forms for two indivi-

duals

were incomplete in that individual item examination

ratings were not recorded

by the examiner.

(2)

The Certificate of Qualification form for one individual

was incorrect in that the individual was recorded

as being

at Level II for one discipline rather than at Level I.

(3)

The record of one individual was incomplete in that it did

not contain documentation

of the required previous experience

of quality control related construction activity.

(4)

The vision examination of one individual had been performed

approximately three

months after the due date.

(5)

The vision examination

record of one individual did not

have the recorded results of a color test.

(6)

. The vision examination record of one individual did not

have complete results

recorded for far distance vision.

The above deficiencies

were corrected

and resolved during the

inveptigation.

The investigators

reviewed audits of QC personnel

qualifications

conducted

by PP8L and Bechtel relating to the period from

October 24,

1975 to July 22,

1976.

These audits identified

items of qualification practices

inconsistent with procedures,

administrative deficiencies

in certification records,

and lack

of orientation training for four individuals.

The investigator

verified that corrective actions

had been accomplished

in

accordance

with QA Program Requirements.

or that the

QC inspectors

were not qualified to perform assigned activities.
