APPENDIX A NOTICE OF VIOLATION Pennsylvania Power 8 Light Company Docket No. 50-387 This refers to the inspect1on conducted by representatives of the Region I (Philadelph1a) off1ce at the Susquehanna Steam Electric Station, Unit l, Berwick, Pennsylvania, of activ1ties authorized by NRC L1cense No.
CPPR-101.
During this inspect1on, conducted on January 30 - February 2, 1979, the following apparent 1tem of noncompliance was ident1fied.
10 CFR 50,. Append1x B, Criterion IX, states, 1n part, that:
"Measures shall be establ1shed to assure that special processes, including welding,...
are controlled and accomplished...
1n accordance w1th applicable
- codes, (and) spec1ficat1ons..."
Section D.3.3.7 of Appendix D of the Susquehanna PSAR states, in part, that:
"Use of qualified procedures and application thereof as required by established codes and standards will be rigidly enforced on all Bechtel
- vendors, subcontractors and Bechtel per-sonnel."
Bechtel Specif1cation 8856-C-66, Revis1on 2, states, 1n part in Paragraph 6.2, that:
"Welding shall be 1n accordance with the
'Structural Weld1ng,'WS Dl.l, Revision 1-76..."
AWS Dl.l, Revision 1-76, "Structural Welding Code," states, in part 1n Paragraph 4.4, that:
"Cracks or blemishes caused by arc strikes shall be ground to a smooth contour and checked to ensure soundness."
Contrary to the above, on February 2, 1979, several arc strikes were observed on structural steel members of the Reactor Building Superstructure and the 11censee could produce no evidence that gA program (weld1ng) controls had been established to properly iden-tify, evaluate, and dispos1tion such arc strikes.
This is an infraction.
