APPENDIX A NOTICE OF VIOLATION Pacific Gas and Electric Company 77 Beale Street, Room 1451 San Francisco, California 94106 Docket Numbers 50-275 and 50-323 License Numbers,DPR-80 and DPR-82 As a result of inspections conducted April 21-25,

1986, and in accordance with the NRC Enforcement Policy, 10 CFR 2, Appendix C, the following violation was identified.

Technical Specification 4.4.6.1.a.

requires that for the Containment Atmosphere Particulate Monitoring System, a "Channel Calibration" be performed every 18 months as specified in Table 4.3-3 to demonstrate that this element of the reactor coolant leakage detection system is operable as required by Technical Specification 3.4.6.1.

Technical Specification 1.5 states that a "Channel Calibration shall be the adjustment, as necessary, of the channel such that it responds within the required range and accuracy to known values of input...."

Contrary to the above requirements, the "Channel Calibration" performed on the Unit 1 Containment Atmosphere Particulate (RE-ll) Monitoring System on February 1,

1986, did not demonstrate the operability of the system.

The calibration failed to verify that the sampling flow rate was accurately indicated by the measuring device and that the high and low flow alarms actuated at the desired flow rates.

The flow rate is used in calculations to determine the high radioactive release rate alarm set point for isolation of the containment ventilation system.

This is a Severity Level IV Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Pacific Gas and Electric Company is hereby required to submit to this office within 30 days of the date"of the letter transmitting this Notice, a written statement or explanation in reply, including:

(1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the 'results

achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending the response time.
