CONSENT AND HEARING WAIVER FORM Entergy Operations, Inc. hereby agrees to comply with the terms and conditions of Confirmatory Order arising out of an alternative dispute resolution process to be issued relating to enforcement action No. 2015-039. The Confirmatory Order will be effective 30 calendar days after its issuance.
By signing below, Entergy Operations, Inc. consents to the issuance of the referenced Confirmatory Order, effective 30 calendar days after its issuance and, by doing so, agrees to waive the right to request a hearing on all or any part of the Confirmatory Order.
