LETTER NUMBER: 2.17.015

Dear Sir or Madam:

In a letter dated March 11, 2016 (Reference 1), Pilgrim Nuclear Power Station (PNPS) responded to the Apparent Violations identified in the U.S. Nuclear Regulatory Commission (NRC) Office of Investigations (Ol)Report No. 1-2015-005 (Reference 2). There were a number of actions specified in the letter that PNPS indicated that it had taken to address the Apparent Violations. In particular, the letter stated an electronic fire watch tracking system was implemented at PNPS.

However, a recent review of the actions identified in the letter has shown that the electronic fire watch system was not implemented at PNPS. Accordingly, the purpose of this letter is to correct our previous response.

In Reference*1, PNPS identified the following specific actions taken by PNPS:

  • Individual instances of missed fire-watches were reviewed and addressed using the station's culpability model with actions taken
  • Fire tour sheets were revised to include additional reviews and approvals
  • A pre-job brief was instituted for fire watch tours
  • Daly shift briefs were implemented which include the specific fire watch assignments
  • An electronic tracking system was implemented

Letter No. 2.17.015 Pilgrim Nuclear Power Station Page 2 of 2 PNPS has performed.a detailed review of the actions taken to address the Apparent Violations identified in Reference 2. As a result of the discrepancy noted above, PNPS verified each of the above referenced actions. In addition to the electronic tracking discrepancy, a discrepancy with approval of the completed fire watch tour sheets was also noted. All other actions were verified to have l:;>een completed as shown .

. In lieu of the electronic tracking system, which was listed as an enhancement action in the Corrective Action Program (CAP), the site implemented a revision to the site fire watch control procedure to incorporate fire watch tour sheets, and to incorporate a requirement to review security door card reader data in order to verify completion of fire watch tours. The following is a revised .list of the actions taken in response to the Apparent Violations identified in Reference 2.

  • Individual instances of missed fire-watches were reviewed and addressed using the station's culpability model with actions taken. (no change)
  • Fire tour sheets were created and site fire watch procedure instruction was updated to

. include additional detail to verify fire watch tour completion via security door card readers.

  • A pre-job brief was instituted for fire watch tours. (no change)
  • Daily shift briefs were implemented which include the specific fire watch assignments.
