ISSUANCE OF A FINAL SIGNIFICANCE DETERMINATION AND NOTICE OF VIOLATION This is to inform the Commission that a Notice of Violation (NOV) will be issued on or about February 27, 2017, to Entergy Operations, Inc. (Entergy), for a violation associated with a White Significance Determination Process finding identified during an inspection of Arkansas Nuclear One. The White finding, an issue of low to moderate safety significance, involved Entergys failure to provide adequate work instructions for maintenance on an emergency diesel generator (EDG) inboard generator bearing oil sight glass which led directly to a failure to provide the bearing with adequate lubrication. Specifically, Entergys failure resulted in the inboard generator bearing oil sight glass being installed upside down after work in November of 2014 to repair a sight glass oil leak. The inverted sight glass resulted in the scribe mark for minimum oil quantity being below the minimum acceptable level for lubrication of the bearing. Inspection indicated that the oil level remained above the minimum acceptable level until June 22, 2016, when the oil in the bearing was changed as a part of normal maintenance. The level of oil replaced in the bearing in June 2016 was determined, post event, to have been adequate to support monthly runs of the EDG. However, the oil level proved to be inadequate when the licensee conducted a 24-hour surveillance run of the EDG on September 16, 2016. The inboard bearing failed due to loss of lubrication, likely caused by oil overheating and vaporization, approximately 14 hours1.62037e-4 days <br />0.00389 hours <br />2.314815e-5 weeks <br />5.327e-6 months <br /> into the 24-hour test. Subsequent inspection concluded that the EDG would have failed to meet its mission time 14 hours1.62037e-4 days <br />0.00389 hours <br />2.314815e-5 weeks <br />5.327e-6 months <br /> into any required run between June 22, 2016 and September 16, 2016.

An NOV is being issued based on Entergys failure to meet the requirements of Technical Specification 6.4.1.a, because the licensee failed to provide adequate work instructions for maintenance on the inboard generator bearing oil sight glass in November 2014 and for the oil change conducted in June of 2016. The period of time analyzed for the risk analysis was 117 days, which is the sum of the period from June 22 to September 16, 86 days, when the emergency diesel generator was degraded, and the repair time of 31 days. Allowance was made in the analysis that the EDG would have run for 14 hours1.62037e-4 days <br />0.00389 hours <br />2.314815e-5 weeks <br />5.327e-6 months <br /> during the 86 day time period prior to bearing failure. In accordance with the NRC Enforcement Policy, this NOV is considered an escalated enforcement action because it is associated with a White finding.

On January 19, 2017, the NRC issued a preliminary determination letter for this issue, provided Entergy the results of the inspection, and offered the opportunity to attend a Regulatory Conference or respond in writing. By phone call to the region on January 20, 2017, Entergy accepted the White finding and violation and indicated no intention to request a regulatory conference or to submit written information on the docket concerning the violation.

It should be noted that the licensee has not been specifically informed of the enforcement action. The schedule of issuance and notification is:
