SUMMARY
On August 9, 2016, a Partial Participation Plume Exposure Pathway Exercise was conducted in the IO-mile Emergency Planning Zone (EPZ) around the FERMI II Nuclear Power Plant (FNPP) by the U.S. Department of Homeland Security (DHS)/Federal Emergency Management Agency (FEMA) Region V. The purpose of this exercise was to assess the level of State and local preparedness in responding to a radiological emergency. This exercise was held in accordance with DHS/FEMA's policies and guidance concerning the exercise of State and local Radiological Emergency Response Plans (RERPs) and procedures.
The most recent exercise at this site was conducted on April 29, 2014. The qualifying emergency preparedness exercise was conducted on February 2, 1982.
The DHS/FEMA wishes to acknowledge the efforts of the many individuals who participated in this exercise. In the State of Michigan, the risk counties of Monroe and Wayne participated along with the State government.
c Protecting the public health and safety is the Partial-time job of some of the exercise participants and an additional assigned responsibility for others. Still others have willingly sought this responsibility by volunteering to provide vital emergency services to their communities.
Cooperation and teamwork on the part of all the participants was evident during this exercise.
This Final After Action Report/Improvement Plan (AAR/IP) contains the evaluation of the biennial exercise and the evaluation of the following out-of-sequence interviews and activities:
State of Michigan Michigan Department of Environmental Quality (MDEQ) at the Monroe and Wayne Counties' out-of-sequence activities demonstration.
Monroe County Public Reception and Decontamination Center/Congregate Care Center at the Whiteford High School.
Public Reception and Decontamination Center/Congregate Care Center at the Bedford High School Wayne County Public Reception and Decontamination Center/Congregate Care Center at the Wayne County Community College.

No Level 1 Findings were identified for any jurisdiction during this exercise.
No Level 2 Findings were identified for the State of Michigan during this exercise.
There was one Level 2 Finding for Monroe County, 23-16-5al-A-Ol.
The faxes that were sent to WJR at 1214 and 1224 hours0.0142 days <br />0.34 hours <br />0.00202 weeks <br />4.65732e-4 months <br />, (received at 1215 and 1227) outlining an Emergency Alert System (EAS) to be broadcast were not written so as to permit radio station personnel to directly read the information to the public. The EAS message required radio station personnel to obtain information from a six-page fax regarding evacuation routes and reception centers. Area 6, another evacuated area, did not have an evacuation route stated in the EAS message. Similarly, the second EAS message contained conflicting information for areas 1, 2, and 4, with no information for areas 6 and 7. The transmitted EAS message also required WJR to broadcast information regarding reception centers. However, the information in the fax was difficult to decipher, as there were notes about whether the reception center was open, and whether to include all reception centers or just the open ones.
Staff at the Monroe County Emergency Operations Center (EOC) did not coordinate with and did not verify with WJR staff when sirens were to sound and when the EAS message was to be broadcast or if radio station staff had any questions. The EAS message was the PAO to evacuate PAA 1, 2, & 6 and had been faxed to the radio station. Although the Emergency Management Director (EMD) called radio station WJR at 1214 hours0.0141 days <br />0.337 hours <br />0.00201 weeks <br />4.61927e-4 months <br /> and confirmed the fax had been received, he did not allow time for the radio station staff to prepare text for broadcast or to record the message before sounding sirens at 1217 hours0.0141 days <br />0.338 hours <br />0.00201 weeks <br />4.630685e-4 months <br />. The radio station staff had difficulty sorting through the six-page fax due to inconsistencies in the information that had been sent.
There were two Plan Issues for the State of Michigan both have been corrected and closed.
44-16-1.e.1-P-01; On February 1, 2016, the new State EOC at 7150 Harris Drive, Dimondale, MI was declared operational. The State EOC Emergency Operations Plans/Job Aids observed during the exercise all bore issue dates of 3/2015 or 4/2015. State EOC SOP# 3, SEOC Setup still contained the 4000 Collins Road, Lansing, MI facility floorplan and procedures. A similar Plan Issue identified during the October 27, 2015 D.C. Cook Exercise when SEOC staff members brought in their own versions of position specific procedures/ job aids and roughly half of the job aid manuals were observed to be left on the cart. Had the outdated references in the job aid manuals been used, confusion and/or errors could have resulted. This Plan Issue has been resolved.
15-15-3.a.1-P-01: The Michigan Department of Environmental Quality (MDEQ) Health Physic Response Team (HPRT) No. 1 Guidance Document Final (7/29/15) indicated that the Canberra MiniRadiac alarms were set at 1,000 mR/hr (low dose rate), 10,000 mR/hr (high dose rate), 800 mR/hr (low dose), and 2,500 mR/hr (high dose). The Guidance Document Final 1 (10/27/15) 7
Unclassified Radiological Emergency Preparedness (REP) Program After Action Report/Improvement Plan FERMI II Nuclear Power Plant indicated that the alarms were set at 1,000 mR/hr (low dose rate), 10,000 mR/hr (high dose rate),
800 mR (low dose), and 2,500 mR (high dose). The NFEMP Update October 2015 Staff Briefing document indicated and the Field Team Center Coordinator stated during his briefing that the MiniRadiac will alarm at 800 mR. The NFEMP Attachments document (7/27115) indicates that notification should take place at 100 mR, 1 R was the administrative limit for 5 rem TEDE dose limit, 2 R for 10 rem, and 5 R for 25 rem. The Decontamination Section used a computer and infrared interface to program the MiniRadiacs to alarm at 1,000 mR/hr (low dose rate), 3,000 mR/hr (high dose rate), 100 mR (low dose), and 1,000 mR (high dose). This Plan Issue has been resolved.

Unclassified Radiological Emergency Preparedness (REP) Program After Action Report/Improvement Plan FERMI II Nuclear Power Plant INTRODUCTION - EXERCISE BASIS On December 7, 1979, the President directed FEMA to assume the lead responsibility for all offsite nuclear planning and response. The DHS/FEMA's activities are conducted pursuant to 44 Code of Federal Regulations (CPR) Parts 350 "Review and Approval of State and Local Radiological Emergency Plans and Preparedness", 351 "Radiological Emergency Planning and Preparedness" and 352 "Commercial 352 "Commercial Nuclear Power Plants: Emergency Preparedness Planning." These regulations are a key element in the REP Program that was established following the Three Mile Island Nuclear Station accident in March 1979.
The Federal Code, 44 CPR 350 establishes the policies and procedures for DHS/FEMA's initial and continued approval of State and local governments radiological emergency planning and preparedness for commercial nuclear power plants. This approval is contingent, in part, on State and local governments' participation in joint exercises with licensees.
The DHS/FEMA's responsibilities in radiological emergency planning for fixed nuclear facilities include the following:
Taking the lead in offsite emergency planning and in the review and evaluation of Radiological Emergency Response Plans (RERPs) and procedures developed by State and local governments; Determining whether such plans and procedures can be implemented on the basis of observation and evaluation of exercises of the plans and procedures conducted by State and local governments; Responding to requests by the U.S. Nuclear Regulatory Commission (NRC) pursuant to the Memorandum of Understanding between the NRC and FEMA dated June 17, 1993 (Federal Register, Vol. 58, No. 176, September 14, 1993); and Coordinating the activities of Federal agencies with responsibilities in the radiological emergency planning process:
o U.S. Department of Agriculture; o U.S. Department of Commerce; o U.S. Department of Energy; o U.S. Department of Health and Human Services; o U.S. Department of the Interior; o U.S. Department of Transportation; 9

Representatives of these agencies serve on the DHS/FEMA Region V Regional Assistance Committee (RAC), which is chaired by DHS/FEMA.
Formal submission of the RERPs for the FNPP to FEMA Region V by the State of Michigan and involved local jurisdictions occurred on November 19, 1981. Formal approval of these RERPs was granted by FEMA on March 9, 1987, under 44 CFR 350.
A REP Partial-Participation, Plume Exposure Pathway Exercise was conducted on August 9, 2016, and evaluated by DHS/FEMA to assess the capabilities of State and local offsite emergency preparedness organizations in implementing their RERPs and procedures to protect the public health and safety during a radiological emergency involving the FNPP.
The purpose of this report is to present the exercise results and findings on the performance of the Offsite Response Organizations (OROs) during a simulated radiological emergency.
The findings presented in this report are based on the evaluations of the Federal evaluation team, with final determinations made by the DHS/FEMA Region V RAC Chairperson, and approved by the DHS/FEMA Headquarters.
The criteria utilized in the FEMA evaluation process are contained in:
NUREG-0654/FEMA-REP-1, Rev. 1, "Criteria for Preparation and Evaluation of Radiological Emergency Response Plans and Preparedness in Support of Nuclear Power Plants," November 1980; FEMA Radiological Emergency Preparedness Program Manual, dated January 2016.
Section 1 of this report, entitled "Exercise Overview", presents information pertaining to the team that planned and coordinated the exercise. This section also provides a listing of all participating jurisdictions and functional entities that were evaluated.
Section 2 of this report, entitled "Exercise Design Summary", contains the purpose and design of the exercise, a description of the plume pathway EPZ and presents basic information and data relevant to the exercise scenario.
Section 3 of this report, entitled "Analysis of Capabilities," presents detailed information on the demonstration of applicable exercise criteria at each jurisdiction or functional entity evaluated in a jurisdiction-based, issues-only format. This section also contains: (1) descriptions of all Level 1 Findings, Level 2 Findings and Plan Issues (if any) assessed during this exercise, recommended corrective actions, and the State and local governments' schedule of corrective actions, if 10

Section 4 of this report, entitled "Conclusion" presents the DHS/FEMA summary of overall exercise conduct and results as evaluated against the requirements of 44 CFR 350.
EMERGENCY PLANNING ZONE (10-Mile EPZ) DESCRIPTION The FNPP is comprised of approximately 1,120 acres of land owned by DTE Energy. The plant operator is the Detroit Edison Company. The site is bounded on the north by Swan Creek, on the east by Lake Erie, on the south by Pointe Aux Peaux Road, and on the west by Toll Road.
Entrance to the site is from Enrico Fermi Drive (a private road owned by Detroit Edison) to the west and from Pointe Aux Peaux Road to another private road owned by Detroit Edison to the south.
The plant is approximately six miles northeast of Monroe, Michigan; 30 miles southwest of downtown Detroit, Michigan; and 25 miles northeast of downtown Toledo, Michigan. The latitude of the site is 41° 57' 48" north and the longitude is 83° 15' 31" west. Site elevations range from the level of Lake Erie, on the eastern edge of the site, to approximately 25 feet above the lake level on the western edge of the site.
The 10-mile EPZ includes parts of Monroe and Wayne Counties in Michigan; parts of Lake Erie; and the southern tip of Essex County, (Bay Point) Canada. Most of the 10-mile EPZ lies within Monroe County. Coordinated efforts occur between the two counties, which are divided into five PAAs. The following jurisdictions are located within the 10-mile EPZ: Ash Township, Carleton Village, Berlin Township, Estral Beach Village, Exeter Township, South Rockwood Village, Frenchtown Township, Monroe City, Monroe Township, Raisinville Township, Brownstown Township, Gibraltar City, Flat Rock City, and Rockwood City. The population in the 10-mile EPZ, based on the 2010 census, is 97,825.
Land use within the 10-mile EPZ is diverse. About 55 percent of the land is farmland. The majority of agricultural land is located in Monroe County. The portion of Wayne County nearest to Detroit is a growing residential suburb. Twelve state parks and recreational areas and several light industries are located within the 10-mile EPZ.
There are three major roads within 10 miles of the plant, Interstates 75 and 275 and U.S. Route
- 24. Their closest approach to the plant is 4.1 miles, 4.2 miles and 5.8 miles, respectively. Five railroad lines operate within IO-miles of the plant. These include the Canadian National Railroad, the Norfolk Southern Railroad, the Chesapeake and Michigan Railroad, the Detroit Toledo and Ironton Railroad and the Detroit and Toledo Shore Line Railroad. There are no major airports within 15-miles of the site.

SUMMARY
2.1 Exercise Purpose and Design The purpose of this exercise is to evaluate Player actions against current emergency response plans, procedures, and performance capabilities for a National Preparedness Program-related incident, and to comply with the requirements of 44 CFR 350 and the guidelines ofNUREG 0654/FEMA-REP-1. Exercise Planners utilized the REP Program Manual (RPM) (January 2016) to develop this exercise.
The objective of the exercise was to demonstrate reasonable assurance that the public can be protected during a NPP emergency.
2.2 Exercise Objectives, Capabilities, and Activities Exercise objectives and identified Capabilities/REP Criteria selected to be demonstrated are discussed in Appendix E.
The Exercise Planning Team (EPT) selected objectives that focus on evaluation of emergency response procedures, identifying areas for improvement, and achieving a collaborative attitude.
This exercise focused on OROs effectively demonstrating the following objectives:
Emergency Operations Center Management Protective Action Decision-Making Protective Action Implementation Emergency Notification and Public Information Support Operations and Facilities 2.3 Scenario Summary Appendix B "2016 FERMI II Nuclear Power Plant Offsite Timeline," contains a summary of the Exercise Scenario, a simulated sequence of events that was used as the basis for invoking emergency response actions by OROs in the FERMI II Nuclear Power Plant Plume Exposure Pathway Exercise for the FERMI II Nuclear Power Plant held on August 9, 2016.
This exercise scenario was submitted by the State Michigan and accepted by DHS/FEMA Region Von July 5, 2016.
During the exercise, in addition to information and data provided through the FNPP onsite scenario, controllers from the State of Michigan provided "inject messages" containing scenario events and/or relevant data to those persons or locations who would normally receive notification of such events. These inject messages were the method used for invoking additional specific response actions by OROs.

Unclassified Radiological Emergency Preparedness (REP) Program After Action Report/hnprovement Plan FERMI II Nuclear Power Plant SECTION 3: ANALYSIS OF CAPABILTIES 3.1 Exercise Evaluation and Results This section presents the results and findings of the evaluation of all jurisdictions and functional entities that participated in the August 9, 2016, REP Partial Participation, Plume Exposure Pathway Exercise to test the offsite emergency response capabilities of State and local governments in the 10-mile EPZ surrounding the FNPP.
Each jurisdiction and functional entity was evaluated based on its demonstration of exercise criteria delineated in the FEMA P-1028 REP Program Manual, dated January 2016. Detailed information on the exercise criteria and the extent-of-play agreement used in this exercise can be found in Appendix E.
Presented below are definitions of the terms used in this report relative to the criteria demonstration status:

3.2 Summary Results of Exercise Evaluation This subsection provides information on the evaluation of each participating jurisdiction and functional entity in a jurisdiction-based, issues-only format.
The matrix presented in Table 3.1, on the following page(s), presents the status of all exercise criteria from the FEMA P-1028 REP Program Manual, dated January 2016, which were scheduled for demonstration during this exercise by all participating jurisdictions and functional entities. The Exercise Criterion status box is blank if it was not scheduled for demonstration.
The DHS/FEMA has developed a standardized system for numbering exercise issues. This system is used to achieve consistency in numbering exercise issues among DHS/FEMA Regions and site-specific exercise reports within each Region. It also is used to expedite tracking of exercise issues on a nationwide basis.
The identifying number of Level I Findings, Level 2 Findings, and Plan Issues includes the following elements, with each element separated by a hyphen (-).
Plant Site Identifier -A two-digit number, corresponding to the Utility Billable Plant Site Code (03 for FERMI II Nuclear Power Plant).
Exercise Year - The last two digits of the year the exercise was conducted.
Demonstration Criterion - The letters and number corresponding to the Demonstration Criterion in the six Exercise Evaluation Areas described in the FEMA P-1028 REP Program Manual, dated January 2016, which supersedes the Federal Register Notice, Vol. 67, No. 80, dated April 25, 2002.
Issue Classification Identifier - (LI =Level 1 Finding, L2 =Level 2 Finding, P =Plan Issue).

PRIOR ISSUES: UNRESOLVED: None 3.3.2.2 Monroe County - Emergency Operations Center Criterion 5.a.1 During the FERMI II Nuclear Power Plant (FNPP) Radiological Emergency Preparedness Exercise on Tuesday, August 9, 2016, Monroe County failed to complete activities associated with primary alerting and notification of the public in a timely manner following the initial decision by authorized offsite emergency officials to notify the public of an emergency situation. The initial instructional message to the public included all minimum elements required by current FEMA REP Guidance. The demonstration was conducted at the Monroe County Emergency Operations Center located at 987 S. Raisinville Road, Monroe, Michigan.
At 0845 hours0.00978 days <br />0.235 hours <br />0.0014 weeks <br />3.215225e-4 months <br />, the Emergency Management Director (EMD) at the Monroe County EOC received notice that an Alert Emergency Classification Level (ECL) had been declared at FNPP at 0837 hours0.00969 days <br />0.233 hours <br />0.00138 weeks <br />3.184785e-4 months <br />. The Executive Secretary for the EOC prepared a message that was faxed to WJR Radio, the Emergency Alert System (EAS) radio station located in Detroit, Michigan. The message informed the station of the event at FNPP a placed the station on standby.
The EOC used pre-scripted EAS messages that are completed by the Executive Secretary with situation specific information and then faxed to the radio station for broadcast. Once the fax was sent, the EMD called the radio station to confirm the fax message was received. After the confirmation process, the sirens were sounded for all or part of the EPZ. Sirens for the entire FNPP EPZ are sounded by the Monroe EOC with coordination from the Wayne County EOC. All siren soundings and radio broadcasts were simulated throughout the exercise.
At 1013 hours0.0117 days <br />0.281 hours <br />0.00167 weeks <br />3.854465e-4 months <br />, the notice was received that a Site Area Emergency (SAE) had been declared at FNPP at 1002 hours0.0116 days <br />0.278 hours <br />0.00166 weeks <br />3.81261e-4 months <br />. And at 1158 hours0.0134 days <br />0.322 hours <br />0.00191 weeks <br />4.40619e-4 months <br />, the notice that a General Emergency (GE) had been declared at 1149 hours0.0133 days <br />0.319 hours <br />0.0019 weeks <br />4.371945e-4 months <br /> was received.
A total of two EAS messages were broadcast during this exercise. The first was a Governor's Protective Action Order (PAO) to evacuate Protective Action Areas (PAA) 1, 2, & 6 and included marine traffic on Lake Erie to five miles. At 1214 hours0.0141 days <br />0.337 hours <br />0.00201 weeks <br />4.61927e-4 months <br />, the EMD faxed the EAS message to 30

At 1208 hours0.014 days <br />0.336 hours <br />0.002 weeks <br />4.59644e-4 months <br />, an amended PAO was received at the EOC that evacuated 1, 2, 4, 6 & 7 and marine traffic to 10 miles from FNPP on Lake Erie. The amended PAO was faxed to the radio station by the EMD at 1224 hours0.0142 days <br />0.34 hours <br />0.00202 weeks <br />4.65732e-4 months <br />. The EMD called WJR to confirm the station had received the second fax and sounded the sirens at 1227 hours0.0142 days <br />0.341 hours <br />0.00203 weeks <br />4.668735e-4 months <br />.
The faxes that were sent to WJR at 1214 and 1224_hours, (received at 1215 and 1227) outlining an Emergency Alert System to be broadcast were not written so as to permit radio station personnel to directly read the information to the public. The EAS message required radio station personnel to obtain information from a 6 page fax regarding evacuation routes and reception centers. Each faxed packet contained the following documents; cover sheet, evacuation order, protective action areas, a map of PAA, routes to reception/congregate care centers for Monroe County, and reception/congregate care centers for Wayne County. The initial EAS request contained information regarding evacuation routes in two places for areas 1 and 2, and they differed in content. Area 6, another evacuated area, did not have an evacuation route stated. in the EAS message. Similarly, the second EAS message contained conflicting information for areas 1, 2, and 4, with no information for areas 6 and 7. The transmitted EAS message also required WJR to broadcast information regarding reception centers. However, the information in the fiix was difficult to decipher, as there were notes about whether the reception center was open, and whether to include all reception centers or just the open ones.
The EAS messaging process from Monroe County EOC to WJR Radio requires that station personnel have a certain amount of training and familiarity with the documents that were sent to prepare for an accurate and timely broadcast. There was little evidence of coordination or instruction from the EOC to the station as to when the sirens would sound and when the EAS message should be read. This lack of coordination could lead to the EAS message being delivered prior to siren alerting that could lead to confusion for public.
Additionally, the message Evacuation Order stated that EAS message was to be broadcast every 15-minutes, while the Monroe County Emergency Plan, Annex D, Appendix 1, Nuclear Accident Procedures Public Information", pp Dl 1 & Dl3, states "(Note: Please rebroadcast this message every five minutes for the first fifteen minutes and then every fifteen minutes until changed or cancelled.)" These are pre-scripted massages for Shelter in Place and Evacuation respectively.
Condition: The faxes that were sent to WJR at 1214 and 1224 hours0.0142 days <br />0.34 hours <br />0.00202 weeks <br />4.65732e-4 months <br />, (received at 1215 and 1227) outlining an Emergency Alert System (EAS) to be broadcast were not written so as to permit radio station personnel to directly read the information to the public. The EAS message required radio station personnel to obtain information from a 6 page fax regarding evacuation routes and reception centers. Area 6, another evacuated area, did not have an evacuation route stated in the EAS message. Similarly, the second EAS message contained conflicting information for areas 1, 2, and 4, with no information for areas 6 and 7. Staff at the Monroe County Emergency Operations Center (EOC) did not coordinate with and did not verify with WJR staff when sirens were to sound and when the EAS message was to be broadcast or ifradio station staff had any questions. The EAS message was the PAO to evacuate PAA 1, 2, & 6 and had been faxed to the radio station. Possible Cause: The possible cause of the EAS process is a lack of coordination between the EOC and radio station as to what and when events should happen. Procedures for the station staff were not available. Documentation sent to the station contained inconsistent information.

Recommendation: Ensure that enough time exists between the decision to activate the sirens and EAS and their actual activation. Coordination between the EOC and radio station as to when sirens are sounded and when the EAS is broadcast is to occur. Staff should review procedures prior to the GE ECL to ensure any preparations that can be made in advance of the decision are done so. Review documents relating to EAS messaging for clarity and remove any unnecessary documents from the fax packet sent to the radio station. Similarly, the second EAS message contained conflicting information for areas 1, 2, and 4, with no information for areas 6 and 7. The transmitted EAS message also required WJR to broadcast information regarding reception centers. However, the information in the fax was difficult to decipher, as there were notes about whether the reception center was open, and whether to include all reception centers or just the open ones.
Staff at the Monroe County Emergency Operations Center (EOC) did not coordinate with and did not verify with WJR staff when sirens were to sound and when the EAS message was to be broadcast or ifradio station staff had any questions.
