SUMMARY

On November 9 & 10, 2016, Radiological Emergency Preparedness (REP) two Medical Services Drills (MS-1) and School Evacuation Drill (EV-2) were conducted by the U.S. Department of Homeland Security (DHS) I Federal Emergency Management Agency (FEMA) for the off-site Medical Services around the Fermi Nuclear Power Plant (FNPP). The purpose of these drills was to assess the level of State and local preparedness for responding to a radiological emergency.

This drill was conducted in accordance with DHS/FEMA's policies and guidance concerning the exercise of State and local Radiological Emergency Response Plans (RERPs) and procedures.

The Beaumont Hospital, South Shore Campus and the ProMedica Regional Hospital in Monroe along with the Monroe Community EMS participated in the drill.

The purpose of the MS-1 Drills was to assess the ability of offsite agencies to respond to a medical emergency involving radiologically-contaminated members of the public. The MS-1 Drills and EV-2 Drill were held in accordance with the DHS/FEMA's policies and guidance concerning the exercise of State and local radiological emergency response plans.

The DHS/FEMA wishes to acknowledge the efforts of the utility and off-site response personnel who participated in the MS-land EV-2 Drills. The risk County of Monroe, as well as various non-government entities and volunteers all contributed to the success of these MS-1 and EV-2 Drills.

Protecting public health and safety is the full-time job of some of the drill participants and an additional assigned responsibility for others. Still others have willingly sought this responsibility by volunteering to provide vital emergency services to their communities. Cooperation and teamwork on the part of all the participants was evident during these MS-1 and EV-2 Drills.

The following criteria, which are part of the six Exercise Evaluation Areas described in the DHS/FEMA Radiological Emergency Preparedness (REP) Program Manual, dated July 2015, were evaluated during the MS-land EV-2 Drills:

  • Criterion l .d. l - At least two communication systems are available, at least one operates properly, and communication links are established and maintained with appropriate locations. Communications capabilities are managed in support of emergency operations;
  • Criterion l .e. l - Equipment, maps, displays, dosimetry, potassium iodide (KI),

and other supplies are sufficient to support emergency operations;

  • Criterion 3.a. l - The Offsite Response Organizations (ORO) issue appropriate dosimetry, KI and procedures, and manage radiological exposure to emergency 5

Emergency workers periodically and at the end of each mission read their dosimeters and record the readings on the appropriate record or chart. The ORO's maintain appropriate record-keeping of the administration of KI to emergency workers;

  • Criterion 3.c.2 - OROs/School officials implement precautionary and/or protective action for schools and;
  • Criterion 6.d.1 - The facility/ORO has the appropriate space, adequate resources, and trained personnel to provide transport, monitoring, decontamination, and medical services to contaminated injured individuals.

No Level 1 Findings were identified for any jurisdiction during this exercise.

No Level 2 Findings were identified for the State of Michigan during this exercise.

INTRODUCTION - EXERCISE BASIS On December 7, 1979, the President directed FEMA to assume the lead responsibility for all offsite nuclear planning and response. The DHS/FEMA's activities are conducted pursuant to 44 Code of Federal Regulations (CFR) Parts 350 "Review and Approval of State and Local Radiological Emergency Plans and Preparedness", 351 "Radiological Emergency Planning and Preparedness" and 352 "Commercial 352 "Commercial Nuclear Power Plants: Emergency Preparedness Planning." These regulations are a key element in the REP Program that was established following the Three Mile Island Nuclear Station accident in March 1979.

The Federal Code, 44 CFR 350 establishes the policies and procedures for DHS/FEMA's initial and continued approval of State and local governments radiological emergency planning and preparedness for commercial nuclear power plants. This approval is contingent, in part, on State and local governments' participation in joint exercises with licensees.

The DHS/FEMA's responsibilities in radiological emergency planning for fixed nuclear facilities include the following:

  • Taking the lead in offsite emergency planning and in the review and evaluation of Radiological Emergency Response Plans (RERPs) and procedures developed by State and local governments;
  • Determining whether such plans and procedures can be implemented on the basis of observation and evaluation of exercises of the plans and procedures conducted by State and local governments; 6

  • Responding to requests by the U.S. Nuclear Regulatory Commission (NRC) pursuant to the Memorandum of Understanding between the NRC and FEMA dated June 17, 1993 (Federal Register, Vol. 58, No. 176, September 14, 1993); and
  • Coordinating the activities of Federal agencies with responsibilities in the radiological emergency planning process:

o U.S. Department of Agriculture; o U.S. Department of Commerce; o U.S. Department of Energy; o U.S. Department of Health and Human Services; o U.S. Department of the Interior; o U.S. Department of Transportation; o U.S. Environmental Protection Agency; o U.S. Food and Drug Administration; and o U.S. Nuclear Regulatory Commission.

Representatives of these agencies serve on the DHS/FEMA Region V Regional Assistance Committee (RAC), which is chaired by DHS/FEMA.

Formal submission of the RERPs for the FNPP to FEMA Region V by the State of Michigan and involved local jurisdictions occurred on November 19, 1981. Formal approval of these RERPs was granted by FEMA on March 9, 1987, under 44 CFR 350.

Two REP MS-lDrills and one EV-2 Drill was conducted on November 9 & 10, 2016, and evaluated by DHS/FEMA to assess the capabilities of State and local offsite emergency preparedness organizations in implementing their RERPs and procedures to protect the public health and safety during a radiological emergency involving the Palisades Nuclear Power Plant.. The purpose of this MS-1 Drills and EV-2 Drill report is to present the drill results and findings on the performance of the OROs during a simulated radiological emergency.

The findings presented in this report are based on the evaluations of the Federal evaluation team, with final determinations made by the DHS/FEMA Region V RAC Chairperson, and approved by the DHS/FEMA Headquarters.

  • NUREG-0654/FEMA-REP-l, Rev. 1, "Criteria for Preparation and Evaluation of Radiological Emergency Response Plans and Preparedness in Support of Nuclear Power Plants," November 1980;

Section 1 of this report, entitled "Exercise Overview", presents information pertaining to the team that planned and coordinated the exercise. This section also provides a listing of all participating jurisdictions and functional entities that were evaluated.

Section 2 of this report, entitled "Exercise Design Summary", contains the purpose and design of the exercise, a description of the plume pathway EPZ and presents basic information and data relevant to the exercise scenario.

Section 3 of this report, entitled "Analysis of Capabilities," presents detailed information on the demonstration of applicable exercise criteria at each jurisdiction or functional entity evaluated in a jurisdiction-based, issues-only format. This section also contains: (1) descriptions of all Level 1 Findings, Level 2 Findings and Plan Issues (if any) assessed during this exercise, recommended corrective actions, and the State and local governments' schedule of corrective actions, if applicable, for each identified exercise issue; and (2) descriptions of unresolved ARCAs and Plan Issues assessed during previous exercises and the status of the ORO's efforts to resolve them.

Section 4 of this report, entitled "Conclusion" presents the DHS/FEMA summary of overall exercise conduct and results as evaluated against the requirements of 44 CFR 350.

EMERGENCY PLANNING ZONE (10-Mile EPZ) DESCRIPTION The FNPP is comprised of approximately 1, 120 acres of land owned by DTE Energy. The plant operator is the Detroit Edison Company. The site is bounded on the north by Swan Creek, on the east by Lake Erie, on the south by Pointe Aux Peaux Road, and on the west by Toll Road.

Entrance to the site is from Enrico Fermi Drive (a private road owned by Detroit Edison) to the west and from Pointe Aux Peaux Road to another private road owned by Detroit Edison to the south.

Unclassified Radiological Emergency Preparedness (REP) Program After Action Report/Improvement Plan FERMI II Nuclear Power Plant The plant is approximately six miles northeast of Monroe, Michigan; 30 miles southwest of downtown Detroit, Michigan; and 25 miles northeast of downtown Toledo, Michigan. The latitude of the site is 4I 0 57' 48" north and the longitude is 83° I5' 3I" west. Site elevations range from the level of Lake Erie, on the eastern edge of the site, to approximately 25 feet above the lake level on the western edge of the site.

The 10-mile EPZ includes parts of Monroe and Wayne Counties in Michigan; parts of Lake Erie; and the southern tip of Essex County, (Bay Point) Canada. Most of the IO-mile EPZ lies within Monroe County. Coordinated efforts occur between the two counties, which are divided into five PAAs. The following jurisdictions are located within the 10-mile EPZ: Ash Township, Carleton Village, Berlin Township, Estral Beach Village, Exeter Township, South Rockwood Village, Frenchtown Township, Monroe City, Monroe Township, Raisinville Township, Brownstown Township, Gibraltar City, Flat Rock City, and Rockwood City. The population in the IO-mile EPZ, based on the 2010 census, is 97,825.

Land use within the 10-mile EPZ is diverse. About 55 percent of the land is farmland. The majority of agricultural land is located in Monroe County. The portion of Wayne County nearest to Detroit is a growing residential suburb. Twelve state parks and recreational areas and several light industries are located within the I 0-mile EPZ.

There are three major roads within 10 miles of the plant, Interstates 75 and 275 and U.S. Route

24. Their closest approach to the plant is 4. I miles, 4.2 miles and 5.8 miles, respectively. Five railroad lines operate within IO-miles of the plant. These include the Canadian National Railroad, the Norfolk Southern Railroad, the Chesapeake and Michigan Railroad, the Detroit Toledo and Ironton Railroad and the Detroit and Toledo Shore Line Railroad. There are no major airports within I5-miles of the site.

SUMMARY

2.1 Exercise Purpose and Design The purpose of this exercise is to evaluate Player actions against current emergency response plans, procedures, and performance capabilities for a National Preparedness Program-related incident, and to comply with the requirements of 44 CFR 350 and the guidelines of NUREG 0654/FEMA-REP-l. Exercise Planners utilized the REP Program Manual (RPM) (January 2016) to develop this exercise.

The objective of the exercise was to demonstrate reasonable assurance that the public can be protected during a NPP emergency.

2.2 Exercise Objectives, Capabilities, and Activities Exercise objectives and identified Capabilities/REP Criteria selected to be demonstrated are discussed in Appendix D.

The Exercise Planning Team (EPT) selected objectives that focus on evaluation of emergency response procedures, identifying areas for improvement, and achieving a collaborative attitude.

This exercise focused on OROs effectively demonstrating the following objectives:

  • Emergency Operations Center Management
  • Protective Action Decision-Making
  • Protective Action Implementation
  • Emergency Notification and Public Information
  • Support Operations and Facilities 2.3 Scenario Summary Appendix E "2016 FERMI II Nuclear Power Plant Offsite Scenario," contains a summary of the Exercise Scenario, a simulated sequence of events that was used as the basis for invoking emergency response actions by OROs in the FERMI II Nuclear Power Plant MS-1 and EV-2 for the FERMI II Nuclear Power Plant held on November 9 & 10, 2016.

This exercise scenario was submitted by the State Michigan and accepted by DHS/FEMA 13

During the drill, in addition to information and data provided through the FNPP onsite scenario, controllers from the State of Michigan provided "inject messages" containing scenario events and/or relevant data to those persons or locations who would normally receive notification of such events. These inject messages were the method used for invoking additional specific response actions by OROs.

Unclassified Radiological Emergency Preparedness (REP) Program After Action Report/Improvement Plan FERMI II Nuclear Power Plant SECTION 3: ANALYSIS OF CAPABILTIES 3.1 Exercise Evaluation and Results This section presents the results and findings of the evaluation of all jurisdictions and functional entities that participated in the November 9 & 10, 2016, REP MS-1 and EV-2 to test the offsite emergency response capabilities of State and local governments in the 10-mile EPZ surrounding theFNPP.

Each jurisdiction and functional entity was evaluated based on its demonstration of exercise criteria delineated in the FEMA P-1028 REP Program Manual, dated January 2016. Detailed information on the exercise criteria and the extent-of-play agreement used in this exercise can be found in Appendix E.

Presented below are definitions of the terms used in this report relative to the criteria demonstration status:

  • M - Met: The status of a REP exercise Evaluation Area Criterion indicating that the participating ORO demonstrated all demonstration criteria for the Evaluation Area Criterion to the level required in the extent-of-play agreement with no Deficiencies, ARCAs or Plan Issues assessed in the current exercise and no unresolved prior ARCAs.
  • Ll -Level 1 Finding: An observed or identified inadequacy of organizational performance in an exercise that could cause a finding that offsite emergency preparedness is not adequate to provide reasonable assurance that appropriate protective measures can be taken in the event of a radiological emergency to protect the health and safety of the public living in the vicinity of a nuclear power plant.
  • L2 - Level 2 Finding: An observed or identified inadequacy of organizational performance in an exercise that is not considered, by itself, to adversely impact public health and safety. Listing of the demonstrated exercise criteria under which a Level 2 Finding was assessed during the current exercise or Level 2 Finding assessed during prior exercises remain unresolved. Included is a description of any Level 2 Findings assessed during this exercise and the recommended corrective action to be demonstrated before or during the next biennial exercise.

3.2 Summary Results of Exercise Evaluation This subsection provides information on the evaluation of each participating jurisdiction and functional entity in a jurisdiction-based, issues-only format.

The matrix presented in Table 3.1, on the following page(s), presents the status of all exercise criteria from the FEMA P-1028 REP Program Manual, dated January 2016, which were scheduled for demonstration during this exercise by all participating jurisdictions and functional entities. The Exercise Criterion status box is blank if it was not scheduled for demonstration.

The DHS/FEMA has developed a standardized system for numbering exercise issues. This system is used to achieve consistency in numbering exercise issues among DHS/FEMA Regions and site-specific exercise reports within each Region. It also is used to expedite tracking of exercise issues on a nat\onwide basis.

The identifying number of Level 1 Findings, Level 2 Findings, and Plan Issues includes the following elements, with each element separated by a hyphen (-).

  • Plant Site Identifier - A two-digit number, corresponding to the Utility Billable Plant Site Code (03 for FERMI II Nuclear Power Plant).
  • Exercise Year - The last two digits of the year the exercise was conducted.

EVALUATION AREA 3 - PROTECTIVE ACTION IMPLEMENTATION Criterion 3.a.1: The OROs issue appropriate dosimetry and procedures, and manage radiological exposure to emergency workers in accordance with the plan and procedures. Emergency workers periodically and at the end of each mission read their dosimeters and record the readings on the appropriate exposure record or chart.

The use of dosimetry and Kl will be demonstrated by ambulance crew. For the purpose of this drill the ambulance Wayne County will simulate issuing dosimetry and Kl to the ambulance crew in lieu of the emergency worker decontamination center.

For purposes of this exercise, if there is no medical need to bring equipment into and out of the treatment room, swabs will be taken at the injury locations and passed out of the room to demonstrate movement of equipment and supplies into and out of the controlled area.

EVALUATION AREA 6.d - TRANSPORTATION AND TREATMENT OF CONTAMINATED INJURED INDIVIDUALS Criterion 6.d.1: The facility/ORO has the appropriate space, adequate resources, and trained personnel to provide transport, monitoring, decontamination, and medical services to contaminated injured individuals.

The hospital will demonstrate procedures for limiting exposure to hospital staff, decontaminating a patient, and restricting access to the area where the patient is being treated and monitored.

Monroe Community EMS will demonstrate the capability to transport a contaminated, injured individual to Beaumont Hospital. The ambulance crew will pick up a contaminated injured patient at Beaumont Hospital Parking Lot (simulated location of accident). Monroe Community EMS staff will utilize universal precautions and good housekeeping practices to minimize the spread of contamination focusing on treating the patient's medical condition.

Monroe Community EMS will call in the information regarding the patient to Beaumont Hospital so they can prepare for receipt of a contaminated patient.

Two hospital staff members will demonstrate proper donning and doffing of full personal protective equipment (PPE). Upon completion of this portion of demonstration PPE requirements may be relaxed with the concurrence of the lead controller and FEMA evaluator. Medical personnel will utilize universal precautions and good housekeeping practices to minimize the spread of contamination, and will focus on treating the patient's medical condition. Simple decontamination efforts will be demonstrated after the patient has been medically stabilized. Hospital personnel will demonstrate their knowledge of who to call for assistance in Radiological Accidents (e.g.,

REAC/TS).

The drill will conclude with hospital representatives demonstrating the removal of protective clothing and surveying of the emergency room and hospital personnel.

Termination will be at the discretion of the Lead Controller, MSP/EMHSD Controller and FEMA Evaluator(s) based on successful demonstration of objectives.

Following the conclusion of the drill, a short critique will be held.
