ML16162A062 Review the incoming report to determine if additional Commission or staff action is warranted. The review should consider whether the report identifies a generic defect or problem with the package design and the safety significance of the issue.
Note that a high safety significance represents a potential for significant radiation exposure, medium safety significance represents a potential for some moderate radiation exposure, and low safety significance represents little or no potential for radiation exposure.
- 1. The report identifies:
__ Significant reduction in the effectiveness of a package during use;

- 3. Summary of the report:
After receipt of a shipment of heels in cylinders, an operator, who was preparing a cylinder for attachment to a process system, the operator discovered that the valve cap did not contain a Teflon cap gasket as expected. The cylinders data book showed that it complied with ANSI N14.1-2001 which in figure 12 shows the required Teflon gasket. Certificate of Compliance No. 9196, Revision No. 29, Condition No. 6 requires that the ANSI standard 30B, 30-inch diameter UF6 cylinder be maintained in accordance with American National Standard N14.1-2012 or an earlier version of ANSI N14.1 in effect at the time of fabrication.
The cylinder valve was verified to be shut by Louisiana Energy Services (LES) and no leakage through the valve was detected.
The licensee attributed human error as the cause of the missing Teflon gasket, since the error occurred prior to receipt at LES.
- 4. Corrective actions taken by the licensee:
As of the date of the report by LES, the cylinder shipper was conducting a causal evaluation to determine the cause and extent of this non-conformance. LES has not had this issue with any other cylinders provided by this vendor.
- 5. Staff comments:
Based on the report, the staff agrees that the safety significance is low and that the gasket omission was likely caused by human error. NRC staff is now aware of any other instances of this occurring with shipments received by LES. There was no indication of a release of UF6 from the package.
- 6. Staff conclusion:
The report does NOT identify generic design or license/certificate issues that warrant additional Commission or staff action. This report is considered closed.
There is a need to take additional action. Provide a summary of the bases and recommended actions:
