PROPOSED IMPOSITION OF CIVIL PENALTY - $50,000 This is to inform the Commission that a Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $ 50,000 will be issued on or about January 25, 1996, to Pacific Gas and Electric Company (PGSE).
This action is based on a breakdown in the licensee's program to assure adequate control of temporary grounding devices.
Specifically, on October 21,
- 1995, several procedures were violated when the licensee failed to remove a "ground buggy" before restoring power to a breaker.
This resulted in a transformer fire and loss of all off-site power supplies to Unit 1.
In determining the
- penalty, the licensee was given credit for corrective actions, but not identification since the violations were discovered after an event and the staff concluded that there were missed opportunities to correct problems in the licensee's program for controlling these devices.
It should be noted that the licensee has not been specifically informed of the enforcement action.
