NRC ILE Outline and Operating Test Submittal Comments 2016 DC Cook Nuclear Plant Units 1 and 2 CE: RD Baker 1 of 7 Chief Examiner Comment Facility Action/Response Outline Submittal Comments Written Exam Outline Duplicate generic KAs between Tier 3 and other two Tiers: 2.1.7, one on RO and two on SRO; 2.2.37, two on RO exam; 2.1.25, 2.4.9, and 2.4.31, one on RO and one SRO.

LIC: Will rework as requested.

NRC: Resolved by replaced 000057 2.1.7 (question 79[SRO]) with randomly selected 2.2.40, and 00WE11 2.2.37 (question 18[RO]) with randomly selected 2.1.20 to preclude oversampling.

Written Exam Rejected KAs Need more information / justification for rejection of RO K/As 000062 AA2.04 & 005000 2.4.3 and SRO KAs 000001 2.1.25 & 000037 2.4.6.

LIC: Explained justifications provided.

NRC: Resolved-justifications adequate.

JPMs-Admin Verify that JPMs for Equip Control & Rad Control are indeed modified. (None are new.)

LIC: Changed the initial conditions for the required isolation boundaries by having different equipment out-of-service (Equip Control).

NRC: Significant change to JPM-therefore modified.

JPMs-System JPM h is from previous (2014) exam, and was initially submitted as Alternate Path; Administered version is not. Which is this one?

LIC: JPM is not Alternate Path, and is therefore not considered as previous use.

NRC: Concur-acceptable. (Only one previous, JPM g.)

Scenario ES-D-1 Scenario 1, Event 9; Scenario 2, Event 6; Scenario 5, Event 8: All these events involve a failure of the MSLs to auto isolate. Both the first two events involve Main Turbine Fails to Auto Trip. Are they different events?

What makes them different?

LIC: Failures initiated by different causes to require different mitigation actions.

NRC: Sat-does not adversely impact required discriminating events for applicant evaluation.

Scenario ES-D-1s Verify that, as a minimum, Scenarios 1, 4, & 5 are new or significantly modified, so each applicant sees at least one new or modified scenario.

LIC: Scenarios 1 & 2 are new and 1 & 3 require EOP contingency actions so that all crews will be exposed to at least one new scenario. (SRO-I crews see 1 and SRO-U crews see 3. Scenario 5 significantly modified.

NRC: Resolved-requirements met.

NRC ILE Outline and Operating Test Submittal Comments 2016 DC Cook Nuclear Plant Units 1 and 2 CE: RD Baker 6 of 7 Chief Examiner Comment Facility Action/Response Scenario D-1 Comments Scenario 1 (2016-01)

1. Ensure that (N) and (R) events are pre-briefed prior to crew entering simulator such that only a quick focus brief prior to event is required during exam.
2. Event 4/5; Items for US state Enters and directs should list the activities that the US is expected to direct.
3. Event 5; The second operator (RO/BOP) action item should be split if the diagnosis (RO) is separate from performance of the immediate action(s) (BOP).

LIC: Clarified reason for pumps swap on turnover.

Added North CRAC to Stop and 30 day time clock to event 3. Cleaned up actions on Event 4 to reflect procedural actions; explained RIL and added notes about Turbine hold Page 10; removed SI actuated check and listed action for No SI and transition to H.1 through red path.

NRC: Resolved-requirements met.

Scenario 2 (2016-02)

1. Event 1; What is/are the significant verifiable operator action(s) for the RO?
2. Event 2; See comment 1 for Scenario 1.
3. Event 3; The second operator (RO/BOP) action item should be split if the diagnosis (RO) is separate from performance of the immediate action(s) (BOP).
4. Major Transient; Expand details of RO/BOP E-0 immediate actions.
5. Major Transient; Expand details of operator actions for E-0, Attachment A (could be separate attachment or section of scenario guide) if significant component failures imposed.

LIC: Event 1 added SM/MTI to actions for relay, added time for TRM -72 hrs. Event 2 removed place in stop since it was previously done. Event 3 Added note about actual failure and step to contact local operator. Event 4 SM/MTI and 6 hr time added to action. Page 7 added alarms to start of event.

NRC: Resolved-requirements met.

Scenario 3 (2016-03)

1. Event 2; See comment 1 for Scenario 1.
2. Event 3; The second operator (RO/BOP) action item should be split if the diagnosis (RO) is separate from performance of the immediate action(s) (BOP).
3. Event 6; Provide an explanation why PORV rad monitors are considered inoperable with the PORV being inoperable.
4. Major Transient; Expand details of RO/BOP E-0/

ECA-0.0 immediate actions.

LIC: Event 2 Added Steps for Reactivity change.

Updated turnover to include Startup steps for power change. Event 3 Made Tasks RO (vs. ATC or RO/BOP). Event 4 added mode change note to TS. Event 5 change Manual to Hand; removed Critical Task for TDAFP start and replaced with Seal Return Isolation. Updated Step for Isolating PZR Heaters on page 11 and drops for checking Bus faults.

NRC: Resolved-requirements met.

NRC ILE Outline and Operating Test Submittal Comments 2016 DC Cook Nuclear Plant Units 1 and 2 CE: RD Baker 7 of 7 Chief Examiner Comment Facility Action/Response Scenario D-1 Comments Scenario 4 (2016-04)

1. Crews 2 and 4 are both slated to see Scenarios 2 and 4. Both scenarios have VCT level instrument failures (one high, one low). While the response actions are different, the same procedures and TS/TRM sections are exercised; therefore a crew should not be administered both scenarios.
2. Similarly, Scenarios 3 and 4 both have failures that cause SG 4 PORV to fail open. Again the operator actions significantly overlap, therefore ensure that a crew is not administered both scenarios.
3. Event 1; Ensure that the simulator provides adequate feedback to ensure that the verifiable operator actions can support a licensing decision.
4. Event 3; See comment 1 for Scenario 1.
5. Event 3; Previous scenarios assign the RO(ATC) the responsibility for turbine manipulation. This scenario assigns the responsibility to either the RO or BOP. Scenarios need to be consistent.
6. Major Transient; Expand details of operator actions for E-0, Attachment A (could be separate attachment or section of scenario guide) if significant component failures imposed.

LIC: Swapped Event 2 from scenario 4 with Event 3 on Scenario 5. Swapped New Event 2 and Event 5 on Scenario 4. Corrected East ESW on event 1. Event 2 and Event 5 updated TS description and times. Added SM/MTI to bistable statement for Event 5 and Event 6. Added after SI to header when RHR trips. Changed P.1 entry to will not may.

NRC: Resolved-requirements met.

Scenario 5 (2016-05)

1. Event 2; See comment 1 for Scenario 1.
2. Items for US that state Enters and directs should list the activities that the US is expected to direct.
3. Major Transient; Expand details of operator actions for E-0, Attachment A (could be separate attachment or section of scenario guide) if significant component failures imposed.

LIC: Swapped Event 2 from scenario 4 with Event 3 on Scenario 5. Swapped Events 2 and Event 3.

Added PR Dev alarms to Event 3. Added SM/MTI to bistable statement for Event 2 and Event 6. Added Drop 31 to Event 5. Added Alarm to CTS Pump trip on page 10, removed IMO 204 from CT, and added Spray Add flow to CT#2.
