VIRGIL C. SUMMER NUCLEAR STATION (VCSNS), UNIT 1 DOCKET NO. 50-395 OPERATING LICENSE NO. NPF-12 SPECIAL REPORT (SPR) 2016-004 South Carolina Electric & Gas Company (SCE&G) is submitting this Special Report pursuant to the requirements of Technical Specifications 6.9.2 and as required by Station Administrative Procedure (SAP) -131A, Fire Protection Program Surveillances and Compensatory Measures,.1, step. 6.1.11, Action 4. This report addresses a fire barrier not being restored to operability status within seven days after declaration of inoperability.
On October 4, 2016, at 18:20 hours, Security/Fire Door DRIB/313, Intermediate Building 436 foot elevation, was declared inoperable after not meeting its surveillance requirements. The surveillance results revealed that the latch was not working properly and the door could not be secured. The failed surveillance was entered into the station's corrective action program. Upon declaring DRIB/313 inoperable, VCSNS implemented an hourly roving fire watch patrol and the door was placarded for non-entry.
The cause of the failure was a damaged lockset. Actions to repair the door consisted of:
replacing the lockset, moving the electric strike up one-half of an inch, filling the gap with weld material, welding new tabs, and replacing the push paddle with a new push bar. The repair was performed in accordance with Civil Maintenance Procedure (CMP) -100.008, Rework or Replacement of Plant Doors.
Repairs were completed on October, 13, 2016, and the door was retested satisfactorily.
DRIB/313 was returned to operable status on October, 13, 2016, at 15:30 hours.
