PRAIRIE ISLAND NUCLEAR GENERATING PLANT, UNIT 2NOTIFICATION OF NRC SUPPLEMENTAL INSPECTION (95001) AND REQUEST FOR INFORMATION

Dear Mr. Northard:

By letter dated March 2, 2016, the U.S. Nuclear Regulatory Commission (NRC) communicated to you its intent to perform a supplemental inspection to review your evaluation of the root and contributing cause(s) and corrective actions for a low-to-moderate (White) performance indicator (PI) for Unplanned Scrams per 7000 critical hours reported in the fourth quarter of 2015 (ML16060A325) for your Prairie Island Nuclear Generating Plant, Unit 2.

On May 23, 2016, your staff notified the NRC of your readiness for it to conduct a supplemental inspection to review the actions taken to address the performance issues.

Therefore, the NRC plans to conduct a supplemental inspection in accordance with Inspection Procedure 95001, Supplemental Inspection for One or Two White Inputs in a Strategic Performance Area, beginning on September 12, 2016. This inspection procedure is conducted to provide assurance that the root cause and contributing cause of risk significant performances issues related to the White PI are understood, the extent of condition and the extent of cause are identified, and the corrective actions are sufficient to prevent recurrence.

In order to minimize the impact to your staff and to ensure a productive inspection for both organizations, we have enclosed a request for documents needed for this inspection. Most of the documents identified are those that the inspectors will need prior to arrival at the plant.

Other documents are requested to be made available to the inspectors upon arrival. It is important that all of these documents are up-to-date and complete in order to minimize the number of additional documents requested during the inspection. If possible, we would request that this information be provided electronically to the inspectors. Any material that is not available electronically should be mailed to the address identified in the enclosure.

We have discussed the schedule for the inspection with your staff and understand that our regulatory contact for this inspection will be Mr. Glenn Carlson of your organization. SUPPLEMENTAL INSPECTION DOCUMENT REQUEST Inspection Report: 05000306/2016009 Inspection Dates: September 12-16, 2016 Inspection Procedure: IP 95001 Please assemble the following documents and provide them to the inspectors on or before the dates requested. Please provide the documents electronically via CD-ROM /DVD format and/or hardcopy. The information sent should be current as of the date of your mailing. Information provided onsite should be the latest approved documentation. The information should be mailed in such a manner as to reach the addressee by the specified date. Proprietary information should be clearly marked and stored on a separate CD-ROM. Draft documentation should not be provided as part of this information request. It is also requested that access to the internet be provided for the inspectors in the room provided to the inspectors.

I. Information Requested on or before September 2, 2016:

1. Administrative procedures governing the sites corrective action program, including:
a. The identification and resolution of problems;
b. Root cause and apparent cause evaluations; and
c. Common cause assessments, effectiveness reviews, and self-assessments;
2. Administrative procedures governing Performance Indicators (PI).
3. The root cause evaluation report(s) and associated documents for the White PI.
4. A copy of all documents referenced in the root cause report(s) for the White PI.
5. A copy of any corrective action documents related to the root cause evaluation report(s).
6. A list of all condition reports associated with PIs since January 1, 2015, including a title or brief description, organized by date.
7. Any internal and/or external assessments of the root cause evaluation(s) and any follow-up assessments that occurred because of the root cause evaluation(s). This would include the results of any Quality Assurance Department assessments, corporate assessments, and mock 95001 assessments conducted at the plant to assess readiness for the inspection.
8. Any internal and/or external assessments of the PI process and the stations efforts to address the White PI.
9. For those documents requested in item 3 and 5 provide:
a. A list of all the corrective actions including the identifying number, short description, and status; Enclosure
b. Documentation supporting completion of any corrective actions;
c. Current schedule and resource commitments for open corrective actions;
d. Schedule and status of any ongoing, planned, or completed effectiveness reviews; and
e. Documentation supporting completion of any effectiveness reviews.

II. Information Requested to be Available on September 12, 2016:

1. Technical Specifications;
2. Updated Safety Analysis Report;
3. Site Organization Charts; and
4. Any previously provided documents that have been updated or revised.
