Mechanical and Civil Engineering Branch (EMCB):
Request for Additional Information (RAil EMCB-1 McGuire Nuclear Station (MNS), Unit 1 and 2, LAR Section 3.2.2, "lnservice Inspection Program for Containment-IWE," provides in Tables 3.2.2-1 through 3.2.2-4 MNS' Second and Third Containment lnservice Inspection (GISI) Interval inspections for both Units 1 and 2, but does not provide specific inspection results for the completed containment inspections associated with the following inspection intervals: For Unit 1, End of Cycle (EOC) EOCs 17-24 refueling outages (RFOs); and for Unit 2, EOCs 17-23 RFOs.
LAR Section 3.2.3, "License Renewal," states that the GISI Plan - IWE program is credited in the joint Catawba and MNS License Renewal Application (LRA) as an aging management program for Reactor Building containment steel components. The program manages loss of material due to corrosion of steel surfaces, and that objective evidence associated with the program's effectiveness was provided in LRA Appendix B.3. 7. Staff's review of the LRA, submitted to the U.S. Nuclear Regulatory Commission (NRC) in 2001, did not identify any specific GISI inspection results to support, other than programmatic, implementation of the program's effectiveness.
Therefore, please provide the results for the completed GISI inspections for each unit as discussed above.
Response to RAI EMCB-1 The tabulation of inspection findings and dispositions for Unit 1, End of Cycle (EOC) EOCs 17-24 refueling outages (RFOs); and for Unit 2, EOCs 17-23 RFOs has been assembled and is presented in Attachment 1 of this response.
RAI EMCB-2 Section 3.4.2 of the LAR discusses the inspection of containment leak-chase channel systems.
NRC Information Notice (IN) 2014-07, "Degradation of Leak-Chase Channel Systems for Floor Welds of Metal Containment Shell and Concrete Containment Metallic Liner," provides three options a licensee may follow to comply with the GISI requirements of Title 10 of the Code of Federal Regulations (10 CFR) Part 50.55a(g)(4).
MNS proposes to add these inspections to the GISI Plan as Elective Examinations to be performed 100% during each Inspection Interval (instead of each Inspection Period, as indicated in the IN). MNS also proposes to perform a VT-3 visual examinatior:i in accordance with procedure NDE-67, "Visual Examination (VT-1 and VT-
- 3) of Metal and Concrete Containment." These examinations may be scheduled and performed as follows:
100% of the containment interior concrete floors shall be examined during each Inspection Interval. Approximately 1/3 of the floor surface areas shall be examined during each Inspection Period to determine the condition of all leak-chase channel bronze caps installed in the floor within the examination area (emphasis added)
2 The examination and schedule proposed by the licensee follows the third option given in IN 2014-07, which states, "Perform insetvice inspection of leak-chase channel system using licensee-proposed alternatives to code requirements under 10CFR50.55a(a)(3)."
Please provide the basis to justify the acceptability of the proposed alternative to that recommended in the IN and the American Society of Mechanical Engineers (ASME) Code requirements (ASME Boiler & Pressure Vessel Code,Section XI, Subsection IWE, Table IWE-2500-1, Examination Category E-A, Item E1.30 "Moisture Barriers," and ASME Code Section XI, Subsection IWE, Subarticle IWE-1240 and Table IWE-2500-1, Examination Category E-C, Item E4.11 "Visible Surfaces").
Response to RAI EMCB-2 In light of the recent issuance of RIS 2016-07, "Containment Shell or Liner Moisture Barrier Inspection," on May 9, 2016, MNS intends to revise the GISI program to perform inspections of Leak-Chase Channel Systems in_ accordance with the periodicity and expectations stated in IN 2014-07, and as amplified in RIS 2016-07.
In response to RAI EMCB-2, the following sections of the LAR have been modified accordingly:
Change 1 Listing of Owner-Specified Examinations:

NRC Information Notice 2014-07 addresses NRC expectations for examination of containment leak chase channel systems. These items shall be added to the Containment ISi Plan to be performed 100% during each Inspection Period. These examinations may be scheduled and performed as follows:
100% of the containment interior concrete floors shall be examined during each inspection period. The floor surface areas shall be examined during each inspection period to determine the condition of all leak chase channel bronze caps installed in the floor within the examination area.
The condition of the bronze caps shall be considered acceptable if there is no evidence of damage or degradation that could result in possible leakage of water into the leakage channel systems.
Evidence of boric acid in the vicinity of any bronze cap or evidence of moisture intrusion into the leak chase channel shall require evaluation by engineering to determine the acceptability of the item.
The specific location Test No. of each bronze cap shall be identified on the examination record.
Change 2 3.4.2 IN 2014-07. Degradation of Leak-Chase Channel Systems for Floor Welds of Metal Containment Shell and Concrete Containment Metallic Liner Extent of Condition Unit 1 One location, #86, was found without a bronze cover during 1 EOC23. Similar to the Unit 2 as found condition, the test port was sealed with a hard solid material obstructing the tube inner diameter. There was a limited extent of condition performed for other test channel enclosures in the area (approximately 10%) with no adverse findings. Unit 1 ILRT data was reviewed with large margin between the latest 2008 ILRT results at 0.1065% per day, and the limit of 0.225%
per day. There is reasonable assurance that all Unit 1 test channel enclosures are covered and/or are otherwise providing an adequate moisture barrier function. An investigation to
3 identify all Unit 1 test channel enclosures was planned for the 1 EOC24 (Spring 2016) refueling outage. This evaluation is applicable and bounding to Unit 1.
Results of 1 EOC24 inspection:
During 1 EOC24, a 100% QC inspection of test channel enclosures was performed. Two unacceptable locations were identified. No. 56 and 143 were both identified with missing caps.
The locations with missing caps were found filled with a concrete like material and were observed to be sealed from moisture intrusion. Filling with grout was performed to return each location to design configuration.
Balance-of-Plant Branch (SBPB):
RAI SBPB-1 The LAR describes and evaluates the ILRT interval extension as being from the 10-year interval to 10.5-years or elsewhere as being approximately 6 months. However, the proposed change to the TS shows a "perform by" being the end of specified refueling outages for each unit (Unit 1
- End of Cycle 26 RFO in spring 2019, Unit 2 end of cycle 25 RFO in fall 2018) rather than calendar dates reflecting the 10 year intervals plus 6 month extensions. While the current schedule/projection of those refueling outages would appear consistent with a six month extension, it is entirely possible that one or both of the identified refueling outages could be delayed in time by a month or more, there being 2-3 or more years between now and the refueling outages when performance is proposed. The App J Option B guidance including model TS wording specifically used a calendar interval of 10 years rather than some number of refueling cycles, since refueling cycle lengths can vary. Extension of the 10-year interval _
described in the guidance was intended primarily to accommodate transitions to longer fuel.
cycle lengths, which were common at the time the guidance was drafted but expected to rarely recur at any particular reactor. Justify how the evaluation bounds or accommodates reasonable delay in the scheduled start dates of the specific refuel outages identified in the proposed TS text.
Response to RAI SBPB-1 Discussion NEI 94-01 Revision 0 (the currently applicable revision of NEI 94-01 to MNS), Section 9.1 states the following regarding the extension of the ILRT interval beyond the current 10 Year interval:
"Consistent with standard scheduling practices for Technical Specifications Required Surveillances, intervals for recommended Type A testing given in this section may be extended by up to 15 months. This option should be used only in cases where refueling schedules have been changed to accommodate other factors."
This amendment request is bounded by the 15-month extension presented in NEI 94-01 Revision 0. The provided analysis as detailed in this LAR adequately covers any slight delays in outage scheduling.
License Amendment Request Request for Additional Information ATTACHMENT 1 RESPONSE TO RAI EMCB-1
- Response to RAI EMCB-1 Mechanical and Civil Engineering Branch CEMCB):
Request for Additional Information (RAil EMCB-1 Response to RAI EMCB-1 Discussion The following is a tabulation of inspection findings and dispositions for Unit 1, End of Cycle (EOC) EOCs 17-24 refueling outages (RFOs); and for Unit 2, EOCs 17-23 RFOs as requested.
1EOC17 Removal of the temporary test bellows, and resulting inadvertent damage to containment vessel sleeve and piping guard pipe, at penetration #M-393.
- 1) That particular portion of the sleeve that was damaged by the saw cuts is not part of containment pressure boundary.
- 2) The containment sleeve does provide reinforcement of the hole in containment where M393 penetrates containment. This hole is still adequately reinforced because the missing material is a small fraction of 1 % of the total sleeve mass. This missing mass is negligible with respect to providing reinforcement of this hole.
- 3) The location and magnitude of these defects where-discussed with a Senior Engineer in the general office and he concurred with this assessment. His only concerns were that these defects have the sharp edges blended off while. minimizing further loss of material. This blending will minimize stress intensification at this location. Additionally, he requested that applicable coating be applied to these areas. A corrective action will be added to complete blending and required coating prior to Mode 4.
These areas will be blended to remove sharp edges and coated as appropriate.
Damage to guard pipe:
The guard pipe is not part the steel containment vessel nor part of a process pipe. The superficial damage to guard pipe is also acceptable without further repair. The minor loss of metal does not interfere with the function of this guard pipe. Per the information in this PIP, the guard pipe damaged areas have already been blended to avoid sharp edges. This blending will minimize stress intensification at this location. All that is required to complete work on the guard pipe is to appropriately coat it. This damage to the guard pipe is not an operability issue. A corrective action will be added to complete required coating prior to Mode 4.
A review*of calculation #MCC-1131.00-00-0003, Volume 2b, page 22 was conducted to determine the impact of the saw cuts on the containment penetration sleeve on the qualification of the containment vessel. If we make the following very conservative assumptions, we can conclude that the sleeve is acceptable and satisfies the original design:
Assumptions:
- 1. The (4) saw cuts are oriented axially on the penetration sleeve
- 2. All (4) saw cuts have a maximum length of 8" and a depth of 5/32"
- 3. All saw cuts lie within an area that is credited for reinforcement of the containment penetration 1
- Response to RAI EMCB-1 opening By review of the calculations, the reinforcement area for the opening is reduced from 46.3 in2 to 45.1 in2 as a result of an area reduction= 5/32" x 8". Because the required reinforcement area is 39.8 in2, adequate reinforcement remains and the penetration sleeve still conforms to the design requirements. Please note that the actual reduction in reinforcement area may be zero because the saw cuts may actually have occurred at a location on the sleeve that is beyond the arec;i replacement limits and therefore would not be credited for reinforcement. However, since the exact location of these saw cuts with respect to the containment shell plate was not recorded, we cannot preclude that some reinforcement may have been removed. In either case, the containment vessel is acceptable. For reasons documented in the detailed problem description, there is no further concern over material removed from the guard piping. No further evaluation is warranted and no additional documentation is necessary.

A VT-1 exam and UT was performed on containment vessel surfaces above and just below the concrete floor level (some concrete was removed to permit this examination). Conditions were found to be acceptable, coatings repairs were completed, and moisture barrier was installed to seal this area from any future moisture.
Action Required: The Containment ISi Plan should be revised to show this area as accessible for visual examination, and the area should be added into Category E-C, Item E4.11 for examination during the next inspection period.

A VT-I exam and UT was performed on containment vessel surfaces above and just below the concrete floor level (some concrete was removed to permit this examination). Description Of Abnormality/Degradation: Cleaning and/or coatings maintenance is needed for the following items:
- 116 degrees/EL. 757' - corrosion on welds under vertical stiffener on top of ring web
- 135 and 166 degrees/ EL. 757' - corrosion on welds under vertical stiffener on top of ring web
- 203 to 220 degreesiEL. 757' - area has been exposed to previous standing water and persistent leakage. Staining observed.
- 203 to 21 O degrees/EL. 761' - penetrations M-320 and M-327 have minor pitting and rust. I
- 234 degrees/ EL. 758' - NS piping penetrations at Shield Bldg. wall have staining from leakage above this area. This appears to be a longstanding issue.
- 13 and 23 degrees/EL. 757' - weld under vertical stiffeners has dirt and paint missing.
These are minor conditions that warrant cleaning and coatings maintenance. Acceptable at this.

Indication Number: 1-SCV0-0005.2007.1 Area/Component l.D.: 1-SCV0-0005 (Ref. Drawings MC-ISIC2-1042-005. -006)
Location/Azimuth: Containment Vessel Exterior Surface - Annulus Ring 5/ 0° through 360°.
Elevation: 766'+5" through 776'+5" Description Of Abnormality/Degradation: The following conditions were observed in 2004 and still exist in 2007: Brown stains cover various surfaces of the stiffener ring web at elevation 767' around the periphery of the containment. These stains are apparently where moisture has dried.
Although it is apparent that moisture had previously covered significant portions of the stiffener ring web, no coatings damage or base metal degradation was observed. O.K. WO# 589092 was written to clean and recoat affected areas. Continue to monitor during future inspections.
Action Required: WO# 589092 has corrected the noted indications.

Sealant along joints in the panel were inspected and found to be in good condition, preventing moisture intrusion behind panel. Work was performed per W0#589422-04. This condition had been identified during 2004.
Resolution: Conditions are considered acceptable. This area should continue to be monitored during future inspections.

At the 9:00 position the lock washer is missing approx. 20% material. At the 7:00 position the lock washer is missing approx. 40% material. Photos are available of this condition.
Resolution: WO 2173920 replaced the degraded washers and any other fasteners and a penetration test was performed after repair-SAT. ACA 1 documents the WR that was rolled into WO 2173920.

In addition, sealant has not been applied along the base of the insulation panel to prevent moisture intrusion. *2EOC16* At the time of inspection on 3/11/05 boron I boric acid crystals were found on the floor, overhead, and on the SS insulation panels underneath the Fuel Transfer Tube radiation shielding concrete.
Resolution: Problem Investigation Process Report PIP #M-00-03292 was written to address this issue. As a result, UT examinations were performed from the opposite side of the Containment Vessel from within the "Labyrinth" area on a limited portion of the exposed area (approximately 6" x 6'-0" long area between Elevation 728'+4" and 728'+ 1 O" approx.). The results of these UT examinations revealed that the thickness containment shell plate has been reduced by no more than 4%. And is considered acceptable at this time.
Reexaminations shall be performed within the next 12 - 18 months to confirm whether degradation is ongoing. The Containment ISi Plan shall be revised to add examination of these areas under Category E-C.
WR# 98148640 was written to clean boron and stains from this area on the insulation panel and floor between azimuths 247° and 270°.
WR # 98148642 was written to add approved sealant along the base of the insulation at the floor between azimuths 247° and 270°. *2EOC16* Engineering was notified and assessed the condition and WR#98340648 was written to clean area. The moisture barrier in this area appeared to be acceptable with no apparent water or moisture intrusion to the liner plate. During this outage UT readings on the liner plate were performed on the annulus side in this zone.

Elevation: 725'+0" Description Of Abnormality/Degradation: (Ref. zone 2-SCVl-0013) Boron/water identified in area under fuel transfer tube (Az. 240-270). This area is covered with boron on the SS insulation panels, on the floor, on the moisture barrier material (2MBRl-004), and on the ceiling coming out at the top edge of the SS panel. This is assumed to be attributed to the longstanding issue concerning the leak in the refueling canal. The SS insulation panel is bulging in one area and the moisture barrier is degrading.
Insulation panel removed and VT-1 performed per W02146423 (Approx. Az. 255deg). UT performed on annulus side per W02096434 (Augmented Item No's: E04.12-0365 through E.04-12.0408) with no measurable wall loss detected.
