Four forms that implement the WCGS RERP have been revised. The revised forms are identified below:

1. Form EPF 06-018-10, "Wolf Creek Phone Team Room Inventory Checklist,"

Rev.10A.

The number of media monitoring team compact discs (CDs) has been changed from five to four. This change is administrative in nature and, therefore, does not decrease the effectiveness of the WCGS RERP.

2. Form EPF 06-018-15, "Emergency Planning Quarterly Communications Checklist," Rev. 13A.

Telephone numbers have been changed for locations listed in this form. This change is administrative in nature and, therefore, does not decrease the effectiveness of the WCGS RERP.

3. Form EPF 06-018-17, "Topeka Phone Team Room Inventory Checklist," Rev.

4A.

The number of media monitoring team CDs has been changed from five to four.

This change is administrative in nature and, therefore, does not decrease the effectiveness of the WCGS RERP.

4. Form EPF 06-001-08, "Off-Site Communicator Checklist," Rev. 1.

A telephone number for the facsimile machine at the Alternate Technical Support Center (TSC) has been added in support of the emergency notification process in the event the Alternate TSC is activated. The name of the contact for one of the owners of WCGS has been changed to Westar Generation Operations.

These changes provide additional guidance for on-shift personnel who perform the Notification/Communication function and, therefore, do not reduce the effectiveness of the WCGS RERP.
