The ESGB staff has finished reviewing the SG tube inspection report (ADAMS Accession No. ML15300A335; dated 10/20/15) and has two questions for you. I dont think these questions warrant a formal RAI, and you can respond via email or letter. For these types of questions, we typically email the questions to the licensee, have a phone call with them to discuss, and write up the discussion in the review. 1. Several indications were reported at, or above, the tubesheet (but below the first tube support plate (TSP) or flow distribution baffle (FDB)). Please confirm that these indications are all a result of wear attributed to loose parts. One indication was identified approximately 0.82 inches below the top of the tubesheet in the tube at row 27, column 97 in SG D during RFO 20. Please discuss the nature of this indication.

Response: A group of fifteen (15) historic indications above the top of the tubesheet in Steam Generator C are monitored each inspection. These indications are all on Row 1 tubes at an elevation of approximately 15 inches above the top of the cold leg tubesheet. The affected tubes are all between columns 4 and 20. Since these indications are all on tubes in the tube lane and at the same elevation, these indications are believed to be due to prior secondary side cleanings, where the tooling wore on the tubes along the tube lane. The indications are being tracked in history and are not growing.

A total of twenty-one tubes throughout the four steam generators contain volumetric indications at the top of the tubesheet. These indication are also monitored each inspection and are not growing. The distribution of these indications is typical of positions where foreign objects have been found to interact with the tubes. None of the indications are associated with a current loose part signal, but it is assumed that the indications are the result of wear from a prior loose part.

Since the indication is slightly below the top of tubesheet, it is believed that this geometric anomaly could have occurred during manufacturing such as expansion into a burr or undercut remaining from the tubesheet drilling process. 2. Please confirm the number of indications reported for anti-vibration bar (AVB), TSP (including FDB), and loose part (LP) wear during RFO 20 in the table below:

  • The table lists 5 indications and 3 tubes for loose part wear in Steam Generator A. The correct numbers are 2 indications and 2 tubes. It appears that the database entries for the Single Circumferential Indication (SCI) in row 20, column 102 may have been included in the table for this category.
  • The table lists 30 indications and 30 tubes for loose part wear in Steam Generator C. The correct numbers are 15 indications and 15 tubes. Fifteen (15) indications in 15 tubes in Steam Generator C are associated with previous wear from secondary side cleaning equipment and are not considered wear from loose parts.
  • The total indications and tubes in the table above are for all indications reported (including the single circumferential indication entries in Steam Generator A and the wear from previous maintenance activities in Steam Generator C). In addition, a few tubes have more than one type of indication in the same tube (i.e.,

a loose part wear indication and wear at an anti-vibration bar location). As a result, the total indications and tubes in the table may differ from the sum of the individual mechanisms.
